The Mounjaro KwikPen: a step-by-step guide to your injection device

Step 1: Before you inject, storage, checks and preparation

Your KwikPen arrives refrigerated and should be kept at 2–8 °C between uses. Many people keep the current pen in the fridge door, where the temperature is stable and it is easy to spot on injection day. The exact window for keeping a pen at room temperature once removed from the fridge is set out in the Mounjaro SmPC on the eMC, always check that document rather than relying on a summary, because the window is specific.

Before each injection, take the pen out of the fridge and allow it to reach room temperature for the time your prescriber or the patient information leaflet recommends. Check the liquid through the viewing window: it should be clear to slightly yellow and free of visible particles. If it looks cloudy, discoloured, or contains floating matter, do not use it. Check the expiry date printed on the label. Wash your hands thoroughly.

Clean the chosen skin site with an alcohol swab and let it dry completely before injecting, injecting through wet skin stings more and can affect absorption. You will need a new needle for each injection; the right needles for the KwikPen are a standard part of the kit to have ready.

Step 2: Attaching a needle and performing the injection

The KwikPen is designed to be straightforward, but the first injection often feels daunting. That is entirely normal, most people find the anxiety fades quickly once they have done it once or twice.

Attach a fresh needle by removing the paper tab, pressing the needle cap firmly onto the pen's cartridge end, and twisting clockwise until secure. Remove the outer needle cap (keep it for disposal) and then the inner cap. You do not need to prime the pen or expel air as you would a traditional insulin syringe.

Pinch the cleaned skin lightly or leave it flat depending on the injection site, then press the pen firmly against the skin until the needle is fully inserted. Press and hold the injection button until the dose counter returns to zero, then continue holding for a further ten seconds before removing. This pause ensures the full dose is delivered. Step 3: After the injection, disposal, records and side-effect awareness

Re-cap the needle immediately using the outer cap, then unscrew and remove it from the pen. Dispose of used needles directly into an approved sharps container, never into a household bin, where they pose a risk to others. The pen itself can be discarded in normal household waste once all four doses are used and the needle has been removed.

Note the date and injection site in a small diary or phone note. Rotating the site systematically (abdomen one week, thigh the next) helps avoid lipohypertrophy (hardened patches under the skin that can reduce absorption). The Mounjaro injection guide covers site rotation in more detail.

The most common side effects after injecting are gastrointestinal: nausea, loose stools, indigestion, or reduced appetite. These are most pronounced after the first injection at a new dose and tend to settle over the following days. According to NHS guidance on tirzepatide, if you experience severe, persistent stomach pain that radiates to the back, seek urgent medical help as this can be a sign of pancreatitis. What the KwikPen does not do, and what that means for your treatment

The KwikPen delivers a fixed dose every time. You cannot adjust the amount per injection, skip part of a dose, or top up if you think something went wrong. If you press the button and the counter does not reach zero, or if the pen leaked, do not inject again to compensate, contact your prescriber.
