What actually happens to muscle when you lose weight on Mounjaro?

Every form of meaningful weight loss (calorie restriction, bariatric surgery, or GLP-1 treatment) tends to reduce some lean mass alongside fat. Muscle loss on Mounjaro follows the same pattern. In the SURMOUNT-1 trial, participants losing around 20% of body weight at the 15mg dose lost a proportion of that as lean tissue, consistent with what's seen in other large weight-loss interventions. The key question isn't whether any lean mass is lost; it's what fraction.

Tirzepatide works partly by reducing appetite substantially, which means total calorie intake drops. When the body runs a sustained energy deficit, it draws on both fat stores and, to a lesser degree, muscle protein. The size of that lean-mass fraction depends heavily on how much protein you eat, how active you are, and how fast weight is coming off. Tirzepatide is not associated with the muscle toxicity risks linked to some other drug classes (statins, for example, carry a rare myopathy risk).
