- I confirm I have answered all consultation questions truthfully and accurately. I understand that incorrect or incomplete information may affect my treatment and put my health at risk, and that NuMe is not liable for any harm resulting from inaccurate information I provide
- I understand this medication must be combined with a healthy diet and increased physical activity.
- I will not take any other weight loss medication at the same time, and I will use this treatment for personal use only.
- NuMe will notify my GP of this treatment in line with GPhC guidelines. I agree to inform any healthcare professional about this treatment before undergoing any surgical procedure, and to update NuMe of any changes to my medication or symptoms during treatment.
- I understand that this medication is not suitable if I am trying to conceive within the next 2 months.
- I will stop this treatment and seek medical help immediately if I experience severe abdominal pain, neck swelling, or changes in mood.
- I understand the prescriber may decide not to issue treatment based on my consultation responses.
- I understand that I must provide a prescription or evidence of treatment from an alternative provider (such as my GP or another pharmacy) if this is my first order and I am requesting a dose higher than the starter dose, or if more than 8 weeks have passed since my last order and I am increasing my dose.
- I give permission for NuMe to access my NHS Summary Care Record or request medical history from my GP if required, and I consent to relevant consultation details being shared with my GP to ensure continuity of care.
- You consent to NuMe accessing your Summary Care Record (SCR) for the purpose of ensuring the safe supply of treatment. If the SCR is unavailable or incomplete, you agree to participate in appropriate two-way communication (e.g. phone, secure message, or video consultation) to verify your medical history. If you do not respond or engage, your order will be cancelled.

Why do we need this?

Here's how it works:
After ordering: You'll get an email to upload:
- Photo ID (passport or driving licence)
- A live video of yourself on your scales, to verify your weight
- Your previous prescription or proof of supply, if you're transferring from another provider
Within 72 hours: If nothing is uploaded within 72 hours of ordering, your order is cancelled and refunded in full, automatically.
