The Mounjaro SmPC, the authoritative product document held on the electronic Medicines Compendium, is unambiguous: store between 2°C and 8°C, do not freeze. That instruction exists because tirzepatide is a biological medicine (a large-molecule peptide) and sub-zero temperatures disrupt the physical structure of the solution in ways a quick visual check cannot reveal. The liquid may look perfectly clear after thawing. It may show no particles, no discolouration, no obvious sign of damage. That appearance is not reassurance; it is exactly the problem. Structural degradation of a biological medicine can occur without any change you can see, and the risk is not a small one: reduced potency at best, an unpredictable injection response at worst. Heat degradation and freeze degradation are different problems. A pen left in a warm car loses potency gradually as the peptide breaks down chemically. Freezing causes a faster, more mechanical disruption: ice crystals form within the solution, expanding and disrupting the suspension, and can also damage the internal components of the KwikPen itself, particularly the cartridge seal. When the ice melts, the damage stays. The solution does not revert to its original state simply by returning to room temperature.
There is also a secondary risk specific to the pen design. Mounjaro's KwikPen is a precision device: the needle, the plunger mechanism, and the cartridge are calibrated to deliver an accurate dose subcutaneously. Ice formation can affect the seal integrity in ways that change the dose actually delivered, even if the pen seems to click and fire normally. This is one reason the SmPC draws a hard line rather than a conditional one, there is no threshold of freezing below which it is deemed acceptable. First, do not inject it. Set the pen aside and do not try to use it on the grounds that it looks fine or that you only have a few doses left.
