The biggest misconception: the starter dose is not the treatment dose

A lot of people start Wegovy at 0.25mg and worry something is wrong when the scales barely move. It feels deflating, especially if you've read about 15% weight loss in clinical trials and expected to see early signs of that. But the 0.25mg dose was never designed to drive weight reduction. Its purpose is entirely different: letting your digestive system adapt to semaglutide before the dose climbs to a level that genuinely suppresses appetite.

Semaglutide slows gastric emptying and acts on appetite-regulating receptors in the brain. At 0.25mg, those effects are mild enough that nausea, vomiting and other gastrointestinal symptoms (the most common side effects) are far less likely to force people to stop. The practical takeaway: if you're at the 0.25mg stage and seeing little happen on the scales, you're not a poor responder. You're on a dose calibrated for tolerance, not treatment.

When do Wegovy 0.25mg results give way to real change?

The standard titration schedule moves from 0.25mg through 0.5mg, 1mg and 1.7mg, spending roughly four weeks at each level, before reaching the 2.4mg maintenance dose. Most people are there by around week 16 to 20. Weight loss tends to become noticeable somewhere in the middle of that climb (typically from 1mg or 1.7mg upward) as appetite suppression grows stronger with each step.

The STEP 1 trial, published in the New England Journal of Medicine, followed 1,961 adults over 68 weeks at the 2.4mg maintenance dose and recorded an average body-weight reduction of around 15%. That figure is what most people are thinking of when they search for Wegovy results. It has nothing to do with the starter dose. For some people, a small amount of early weight loss does occur at 0.25mg (possibly from reduced appetite or adjustments to eating habits) but this is not reliably produced at that dose and should not be expected. A prescriber, not a schedule, decides when your dose increases.

Side effects at 0.25mg and what to watch for

The good news about the starter dose is that side effects are usually at their lightest here. Nausea is the most reported issue across the Wegovy programme; at 0.25mg many people experience it only mildly, if at all. Constipation, loose stools, burping and some fatigue are also possible, the fatigue pattern during titration is worth knowing about before you reach the higher doses.

One thing worth flagging: severe, persistent abdominal pain that radiates toward the back needs prompt medical attention. The MHRA's Drug Safety Update communications have highlighted acute pancreatitis as an infrequent but serious risk with GLP-1 medicines, including semaglutide. It is rare, but the symptom pattern is worth knowing.
