Why do all Wegovy pens contain 3 ml, regardless of the dose?

The 3 ml fill is a manufacturing and device standard, not a clinical measurement. Novo Nordisk designed the Wegovy KwikPen to hold 3 ml of liquid, which provides four weekly injections before the pen is empty. What varies across the dose range is the concentration of semaglutide dissolved in that 3 ml. A 0.25 mg weekly pen contains a much lower concentration per ml than a 2.4 mg weekly pen, even though both hold exactly the same total volume.

Storing the pen correctly matters too. Wegovy pens should be kept refrigerated between 2°C and 8°C until you are ready to use one. What are the actual dose steps, and how does titration work?

Because the 3 ml volume is fixed, the titration schedule moves through different concentration pens rather than adjusting how much liquid you inject. Treatment starts at 0.25 mg per week for the first four weeks, a tolerability stage designed to let your body adjust, particularly to the gastrointestinal effects that are common early on. From there, doses increase roughly every four weeks: 0.5 mg, then 1.0 mg, then 1.7 mg, and finally the standard 2.4 mg maintenance dose.

The MHRA's April 2026 approval of a single-dose 7.2 mg pen (confirmed on the GOV.UK announcement) extends the ceiling further for eligible patients. That higher dose is for adults with a BMI of 30 or above and is reached only after working through the lower rungs under clinical supervision. The starting dose remains 0.25 mg regardless of which maintenance dose you ultimately reach.
