The NICE recommendation for semaglutide (Wegovy) (published as Technology Appraisal TA875) makes clear that this is a medicine intended to be used alongside an appropriate diet, increased activity, and clinical monitoring. That framing matters when you're pricing things up, because Wegovy's clinical profile was built around supervised use. The STEP 1 trial, published in the New England Journal of Medicine, reported an average body-weight reduction of around 15% over 68 weeks at the 2.4mg maintenance dose. Those results came from participants in structured programmes, not from people who sourced a pen cheaply and carried on as before.
What this means in practice: the clinical structure around the medicine is part of what makes it work. A prescriber who checks your starting weight, reviews your history, and re-assesses before each repeat isn't a bureaucratic hurdle. They're a working part of the treatment. Any price that strips that out hasn't made Wegovy affordable, it's removed the thing that matters.

The MHRA has warned publicly about fake semaglutide and tirzepatide pens sold online and through social media, including cases where counterfeit pens were found to contain insulin. In October 2025, the first UK manufacturing site producing counterfeit tirzepatide pens was raided in Northampton, following enforcement action that uncovered around 20 million doses of illegally traded weight-loss medicines with an estimated street value of £45m.
