The reality of eating on Wegovy, from what the discomfort means to when to seek help

The myth: overeating on Wegovy means the medicine isn't working

This is the misconception our prescribers hear most often. People start Wegovy expecting appetite to disappear entirely, so when they find themselves at a birthday dinner or a Sunday roast having eaten far more than planned, they assume something has gone wrong. It hasn't. Semaglutide works by activating GLP-1 receptors that slow stomach emptying and signal fullness to the brain, but it operates in the background of a system that is still influenced by habit, emotion, social cues and the sheer palatability of certain foods. No medicine overrides all of that.

What the evidence does show is a consistent, meaningful reduction in average food intake across a population over time, the STEP 1 trial, published in the New England Journal of Medicine, reported around 15% average weight loss over 68 weeks at the 2.4mg maintenance dose. That figure reflects weeks and months of generally reduced intake, not a complete absence of overeating occasions. One meal does not undo the pharmacology.

What actually happens in your body when you eat too much on Wegovy

This is where the discomfort comes from, and it's worth understanding clearly. Semaglutide slows gastric emptying as part of its mechanism, food moves from your stomach into the small intestine more slowly than it would without the medicine. When you eat a large or rich meal on top of that already-slowed process, the stomach has to accommodate considerably more volume than it comfortably can. The result is often pronounced bloating, nausea, reflux, or a heavy, full sensation that can last several hours.

Vomiting is possible after a very large meal, particularly in the early weeks of treatment or after a dose increase, when GI side effects are at their most noticeable. The NHS patient information for semaglutide lists nausea, vomiting, diarrhoea, constipation and indigestion as common side effects, most pronounced at the start and after each upward titration step. Eating a large meal amplifies these effects rather than creating new ones.

Should you skip your next injection after overeating?

No. This is a question that seems logical but rests on a misunderstanding of how the medicine works. Wegovy is injected once weekly and reaches a steady state over several weeks of consistent dosing. Skipping or delaying a dose because of one meal disrupts that steady state without offering any benefit, and dose decisions (including timing adjustments) are always made by your prescriber, not by the patient responding to a single event.

If overeating caused vomiting and you are uncertain about your hydration, that is the thing to manage: small, frequent sips of water or an oral rehydration solution. Severe vomiting that prevents you keeping fluids down for more than 24 hours should prompt a call to your GP or clinical team rather than a decision to adjust your Wegovy schedule yourself.

When to contact your clinical team

Most episodes of eating too much on Wegovy resolve on their own within hours and need nothing more than rest and hydration. There are situations, though, where you should get medical help rather than wait it out at home.

Contact your prescriber or GP promptly if you experience severe stomach pain that extends towards your back, with or without vomiting, this can be a sign of pancreatitis, which the MHRA identified in a Drug Safety Update for GLP-1 medicines as an uncommon but serious side effect requiring urgent assessment. Also seek help for signs of significant dehydration (dark urine, dizziness, inability to keep fluids down), an allergic reaction, or any symptom that feels out of proportion to a meal.
