The best place to put a Wegovy injection — and why it matters

Picture your first injection day: where are your hands going?

You have opened the plain outer box, the pen is in your hand, and the question nobody warned you about arrives: exactly where do I put this? The abdomen is the site most people start with, and for good reason. There is usually a generous area of soft fatty tissue, it is easy to reach without a mirror, and you can watch what you are doing. Use the area below your waistband and above your hip bones, working in a rough grid of spots and leaving at least two finger-widths between each one. The 5 cm zone directly around your navel is out, the tissue there tends to be thinner and more sensitive.

The front of the thigh is the next most practical option, especially if abdominal soreness is building. The outer two-thirds of the thigh, roughly mid-way between the knee and the hip, gives plenty of soft tissue. Sit down when you use this site; it relaxes the muscle underneath, which makes pinching the skin easier.

The upper arm (specifically the back of it) works well but usually needs a second pair of hands unless you have practiced reaching around. Why rotating spots is not optional

Using the same square centimetre every week creates a cumulative problem. The fatty tissue thickens, hardens, and can form a lump called lipohypertrophy. That lump looks harmless but it changes absorption: medicine injected into fibrous tissue enters the bloodstream unpredictably, which can blunt the effect of your dose or create peaks and troughs you would not otherwise experience. This is well-documented with subcutaneous medicines generally, and semaglutide is no different.

Rotation solves it. The simplest system is a mental clock-face on each site: if last week was 12 o'clock on your left thigh, this week is 3 o'clock, and so on. You cycle around the face, then switch to the right thigh or the abdomen before that site also accumulates too much use. Some people keep a simple note in their phone, one line per dose, one letter for the site. It sounds fussy until you have been on treatment for six months and realise you cannot remember where the last three went.

Site prep and what to watch for afterward

Clean the skin first. An alcohol swab wiped across the chosen spot and left to dry for a few seconds is all that is needed, if you want a convenient supply, alcohol swabs are available from our pharmacy. Dry means dry: injecting through wet alcohol stings. Pinch a fold of skin with your non-dominant hand, insert the pen as the instructions describe, release the pinch only after you have pressed the button and counted to ten. That pause matters; releasing early can pull the medicine back out before it has dispersed.

After the injection, do not rub the site. A firm press with a finger for a few seconds is enough. Rubbing distributes the medicine unevenly and can increase local irritation.
