Start journey Learn moreMoving up to the 5mg dose means your 2.5mg pen has done its job and your prescriber is satisfied with your tolerance so far. Mounjaro is a prescription-only medicine; the only lawful route to a step-up dose in the UK is a clinical assessment by a qualified prescriber, who will confirm it is appropriate for you before anything is dispensed. At nume, that review happens on the day you submit your request — a real clinician, not automated software, reads your case. Here is exactly what that process looks like.

The titration schedule for tirzepatide starts at 2.5mg for the first four weeks. That opening dose is designed to let your body adjust: it reduces the likelihood of nausea and other gastrointestinal effects before the therapeutic work begins in earnest. After those four weeks, if you have tolerated the starter pen well, a prescriber can move you to 5mg. That is not a decision you should make alone. A prescriber looks at how you have responded, what your weight trend shows, and whether anything has changed in your health picture since you started. Our Mounjaro overview covers how the full titration programme is structured, which is worth reading if you are new to the journey. The NICE appraisal of tirzepatide (TA1026) and the NHS patient guidance both note that continuing treatment is appropriate only when clinical review supports it, dose increases are no different.
For anyone using a private online pharmacy, the process of stepping up begins with a clinical questionnaire that your prescriber will review. At nume, transfer patients and those requesting a dose increase must provide evidence of their current treatment, this is a safety requirement, not a formality. A GPhC-registered prescriber on our clinical team then assesses the submission personally. If they are satisfied that 5mg is appropriate, a prescription is issued and the order moves to our GPhC-registered pharmacy for dispensing. The GPhC register entry for our pharmacy (registration 9012878) is publicly verifiable; we encourage you to check it. If you want to understand how we handle the initial prescription route more broadly, this page on buying Mounjaro online in the UK sets out the full picture.
The MHRA has issued repeated warnings about counterfeit weight-loss pens sold through social media and unofficial websites. In October 2025, enforcement teams raided the first illicit UK site manufacturing fake tirzepatide pens, and across 2025 roughly 20 million doses of illegally traded weight-loss medicines (around £45m street value) were seized. Fake pens have contained insulin and other unknown substances. The only safe route is a pharmacy you can check on the GPhC register. Red flags include sellers offering these medicines without a prescription, prices far below the market range, and unknown online sellers pushing discount codes. If you encounter a suspect seller, you can report them directly via the MHRA Yellow Card scheme. The next step-up dose page carries the same verification guidance for when you reach that point.
Once dispatched, your pen arrives the next working day via DPD tracked delivery. A quick note on timing: if you are planning ahead around a bank holiday or a busy period at the end of the month, ordering before the 12pm weekday cut-off on the last working day before a break avoids any gap in treatment. The 5mg pen follows the same injection routine as the starter pen, same sites (abdomen, thigh or upper arm), rotated each week, and if you are ready to order you can buy the Mounjaro 5mg pen directly through our pharmacy. Storage is refrigerated at 2–8°C; the exact room-temperature window is in your Patient Information Leaflet and should be followed precisely. Side effects at this dose can include nausea, loose stools or indigestion, particularly in the first week or two as your system adjusts. The NHS tirzepatide medicine page lists common and less common effects clearly. If you experience severe, persistent stomach pain that radiates to the back, seek medical help promptly. After 5mg, the titration path continues: our pages on stepping up to 10mg and moving to 12.5mg follow the same clinical logic. For a broader look at treatment options alongside Mounjaro, our weight-loss treatments overview is a useful reference. Any questions between doses can go to our prescribers via the support page, aftercare is included, seven days a week. Patients collecting locally can also find out more about visiting our Mounjaro service at Wells Pharmacy.
