This is the question our prescribers hear most weeks, and the answer is more layered than most people expect. Replacement pen needles sold as accessories (the fine-gauge tips that screw onto the KwikPen) can be purchased without a prescription, because the needle itself is not the medicine. The Mounjaro KwikPen, however, is a prescription-only medicine under the Human Medicines Regulations 2012. In October 2025, enforcement agencies raided an illicit UK manufacturing site producing fake tirzepatide pens; across 2025 approximately 20 million doses of illegally traded weight-loss medicines (around £45 million in street value) were seized. Some fake pens seized earlier contained insulin rather than the labelled medicine, leading to hospitalisations.

Mounjaro is licensed for adults with a BMI of 30 or above, or a BMI of 27 or above alongside at least one weight-related condition such as high blood pressure, type 2 diabetes, high cholesterol, or obstructive sleep apnoea.
