Mounjaro (tirzepatide) holds a UK licence for weight management in adults with a BMI of 30 or higher, or in adults with a BMI between 27 and 29.9 who have at least one weight-related condition, for example, high blood pressure, type 2 diabetes, high cholesterol, obstructive sleep apnoea or cardiovascular disease. Tirzepatide is the only dual GIP and GLP-1 receptor agonist licensed for weight management in the UK, a distinction that shaped the clinical programme behind it. The SURMOUNT-1 trial, involving 2,539 adults with obesity and published in the New England Journal of Medicine, recorded average body-weight reductions of around 20–21% at the 15mg dose over 72 weeks, the scale of evidence that underpinned NICE's recommendation in TA1026.
NICE recommends tirzepatide for NHS use in adults with a BMI of 35 or above plus at least one weight-related comorbidity. The private licensed threshold is lower, at BMI 30 (or 27 with a qualifying condition), which is why many people who don't yet meet NHS criteria still qualify through a private prescriber. One group for whom Mounjaro is not licensed: under-18s. It is also not recommended during pregnancy, breastfeeding, or when actively trying to conceive. These exclusions are firm, and a prescriber will ask about them directly during the assessment.
Mounjaro is a Prescription-Only Medicine (POM) under the Human Medicines Regulations 2012. That means it cannot legally be supplied without a valid prescription issued following a genuine clinical assessment. There is no legal shortcut. The practical route (whether NHS or private) always runs through a prescriber.
On the NHS, access is subject to phased rollout criteria that are stricter than the licensed thresholds and often come with significant waiting times. Through a regulated private online pharmacy, the process runs roughly as follows: you complete a clinical consultation (health history, current conditions, medicines you take); a prescriber reviews it; if appropriate, a prescription is issued and your treatment is dispensed and sent. Beyond the register check, the MHRA has repeatedly warned the public about counterfeit weight-loss pens sold online and via social media. In October 2025, enforcement action uncovered the first illicit UK manufacturing site producing counterfeit tirzepatide pens, in Northampton, and approximately 20 million doses of illegally traded weight-loss medicines, worth around £45 million, had already been seized in the preceding period. Fake pens have, in documented cases, been found to contain insulin rather than the stated medicine. The warning signs are consistent: sellers offering these medicines without a prescription, unusually low prices, discount codes, and no verifiable regulatory registration.
