Can I get the new weight loss injection — and do I qualify?

Maybe a friend mentioned it, or you read about clinical trial results and thought: could this be for me? That curiosity is reasonable. The newer weight loss injections (particularly tirzepatide, sold as Mounjaro) have produced results in trials that genuinely surprised the research community. In SURMOUNT-1, published in the New England Journal of Medicine, participants at the highest dose saw an average body-weight reduction of around 20–21% over 72 weeks. Those aren't numbers the field was used to seeing from a medicine.

But the headline figures come from a specific population: adults with obesity and no type 2 diabetes, assessed and monitored throughout by clinicians. The results belong to a supervised treatment programme, not to a pen bought casually. That's the context worth holding onto before asking how to access one.

The legal position is clear. In the UK, both Mounjaro and Wegovy are Prescription-Only Medicines under the Human Medicines Regulations. A prescriber must review your health, weight, medical history and any medicines you already take before authorising treatment. That review isn't a hurdle designed to put you off, it's what makes the medicine safe for you, and what keeps your treatment appropriate as your dose changes over time.

Who actually qualifies, and why BMI isn't the whole story

The licensed eligibility criteria for both injections are broadly similar. A BMI of 30 or above qualifies on its own. A BMI of 27 to 29.9 can also qualify if you have at least one weight-related condition, high blood pressure, high cholesterol, obstructive sleep apnoea, prediabetes, type 2 diabetes, and osteoarthritis are among the conditions recognised. For some South Asian, Chinese, Middle Eastern, Black African and African-Caribbean backgrounds, UK guidance applies thresholds that are 2.5 kg/m² lower than the standard figures.

You can get a rough sense of where you stand using the NHS BMI calculator, but BMI is the starting point, not the verdict. A prescriber will also look at your complete medical history, any contraindications (certain thyroid conditions, a history of pancreatitis, and others listed in the product information), and medicines you currently take. One thing worth saying plainly: qualifying on paper still means a prescriber makes the final call. That's not a loophole, it's the point. The same clinical rigour that produces those trial results requires proper individual assessment.

The NHS route, the private route, and what each involves

The NHS does prescribe these medicines, but access is phased and tied to strict criteria. As of summer 2026, NHS tirzepatide is available to adults with a BMI of 40 or above alongside four or more qualifying conditions; a second cohort covering BMI 35–39.9 with four or more conditions became eligible from June 2026. Further cohorts are planned through 2027. Every NHS patient also needs to engage with a structured diet and activity support programme alongside the medicine. One more thing before you decide

These medicines have a well-characterised side-effect profile, mostly gastrointestinal, nausea, loose stools, and similar symptoms are common in the early weeks, particularly around dose increases, and usually settle.
