Start journey Learn moreGoing from 0.25mg to 1mg Wegovy in a single step isn't part of the licensed dosing schedule — and for good reason. The standard pathway moves in four-weekly steps: 0.25mg, then 0.5mg, then 1.0mg, so your body has at least eight weeks of adjustment before reaching 1mg. Skipping the 0.5mg stage isn't clinically recommended, because the semaglutide (Wegovy) titration schedule is designed specifically to limit the gastrointestinal side effects that tend to hit hardest at each new dose level. Wegovy is a prescription-only medicine, and your prescriber (not you) decides when and whether a dose increase is appropriate.

The first pen of Wegovy exists to let your body settle into semaglutide, not to produce weight loss. At 0.25mg you're unlikely to notice a dramatic change in appetite, and that's entirely expected. The starting dose keeps early side effects manageable while your gut adapts to a GLP-1 receptor agonist slowing gastric emptying and adjusting hunger signals. Many people find the first four weeks the most reassuring part of treatment precisely because the medicine is gentle at this point. Your 0.25mg Wegovy dose is a foundation, not a plateau.
Four weeks at 0.25mg is the minimum before any increase is considered. Staying longer than four weeks on this dose is sometimes clinically appropriate, for instance, if you've had a difficult patch with nausea, or if a planned procedure or holiday means the timing of a dose change isn't ideal. A fortnight away in August, a busy period at work, a payday week when your routine is already disrupted: there are perfectly ordinary reasons to hold a dose, and none of them mean treatment has failed. The NHS semaglutide medicines guide is clear that titration is guided by how well you're tolerating the medicine.
Moving from 0.25mg to 1mg directly bypasses the 0.5mg step that the licensed schedule requires. This matters clinically. Semaglutide's GI effects (nausea, loose stools, stomach discomfort) are closely tied to the size of each dose jump. The 0.5mg stage halves the size of that eventual step to 1mg, making the transition far more manageable for most people. If you were to jump straight to 1mg, you'd be taking on a four-fold increase in dose in one go. That isn't how the medicine is approved to be used, and a prescriber following the Wegovy SmPC on the electronic medicines compendium (eMC) would not advise it.
The 0.5mg step also serves a diagnostic purpose: it shows how your body responds to a therapeutic dose of semaglutide before going higher. Some people find 0.5mg brings meaningful appetite suppression; others move steadily toward 1mg without difficulty. Either experience informs what happens next. You can read more about what to expect during the move from 0.25mg to 0.5mg if you're still on the starter pen.
Once four weeks at 0.5mg have passed and your prescriber is satisfied you're tolerating that dose, you can move to 1.0mg. For most people this is where appetite suppression becomes noticeably stronger. The 1mg dose is neither the ceiling nor a resting point; the pathway continues upward. At each subsequent step (1mg to 1.7mg and then 1.7mg to 2.4mg) the same four-week minimum applies, and the prescriber reviews suitability each time.
It's worth knowing that some people find a dose that works well for them before reaching the maximum. The concept of a maintenance dose that suits an individual is sometimes called a golden dose. Whether that's 1mg or a higher level is a clinical conversation, not something to pre-decide before treatment starts. The full picture of the Wegovy dose schedule sets out all five stages and what each one involves.
Side effects at 1mg can differ from earlier stages, and knowing what's typical helps you report anything unusual to your prescriber promptly. The side effect profile at 0.5mg offers a useful comparison point, since GI symptoms at 1mg often resemble (but can be somewhat more pronounced than) those at the previous step. A yellow card report can be submitted to the MHRA at yellowcard.mhra.gov.uk if you experience anything unexpected.
If you're exploring your options for starting Wegovy through a regulated private provider, the weight-loss treatment consultation at nume connects you with a GPhC-registered prescriber who reviews your case personally before any prescription is issued. Every dose increase goes through that same clinical review.
