Can Mounjaro Cause Vaginal Dryness?

What Mounjaro's clinical programme tells us — and what it doesn't

The SURMOUNT-1 trial, published in the New England Journal of Medicine, randomised 2,539 adults with obesity (without diabetes) to tirzepatide or placebo over 72 weeks. Adverse events were tracked systematically. Vaginal dryness, vaginal atrophy or related genitourinary symptoms were not identified as signals in that dataset or in subsequent regulatory review by the MHRA when granting Mounjaro its UK licence for weight management. The UK Summary of Product Characteristics on the eMC lists the side effects identified across the SURMOUNT programme; genitourinary dryness is not among them.

That absence matters, but it is not the complete answer. Clinical trials capture effects that reach a threshold of statistical significance across a population. Symptoms experienced by a smaller proportion of participants (or symptoms whose cause is indirect) can sit below that threshold even when real for the individuals involved. So the evidence says: tirzepatide does not appear to cause vaginal dryness directly through its pharmacological mechanism. It does not say no woman will ever notice this during treatment.

Why weight loss itself might be the more likely explanation

This is the part that genuinely surprises some people. Oestrogen is produced not only by the ovaries but also by adipose (fat) tissue, via a process called peripheral aromatisation. When body weight falls substantially (as it can on tirzepatide, with average reductions of around 20% at the highest dose in clinical trials) peripheral oestrogen production falls alongside it. For pre-menopausal women this effect is usually modest; the ovaries remain the dominant source. For peri-menopausal or post-menopausal women, who already have lower baseline oestrogen, a further reduction from fat-tissue loss can be more noticeable.

Low oestrogen is a well-established driver of vaginal atrophy and dryness. So if vaginal dryness develops or worsens during Mounjaro treatment, the weight loss itself is a more plausible biological explanation than a direct drug effect. The same can happen with any significant weight reduction, including through dietary changes alone.

This is worth raising with your GP or prescribing clinician rather than putting up with it. Localised oestrogen treatments, vaginal moisturisers and other options are effective and widely available on the NHS, none of them interact with tirzepatide in any documented way. NHS England's guidance on weight-management injections advises discussing HRT choices (particularly transdermal preparations) with your doctor while on tirzepatide, which is related context worth knowing.

Contraception, HRT and the absorption question

One thing the prescribing guidance is clear on: tirzepatide slows gastric emptying, which can reduce the absorption of oral medicines taken at the same time. For women using oral contraceptive pills, the MHRA and NHS guidance specifically advise adding a non-oral method (condoms, for example) for the first four weeks of treatment and for four weeks after every dose increase. This does not mean the pill stops working entirely, but the precaution is there for good reason.

For women using oral HRT, the same absorption consideration applies. NHS England recommends discussing a switch to a transdermal form (patches or gels, which bypass gut absorption) with your doctor.
