The evidence on semaglutide, uric acid and joint pain — what we actually know

Is gout a recognised side effect of semaglutide?

Looking at the UK prescribing information for Wegovy, gout does not appear in the list of side effects, not in the common category, not in the uncommon one. The side-effect profile that consistently emerges from clinical trials is gastrointestinal: nausea, diarrhoea, constipation, indigestion and fatigue account for the vast majority of what people report, particularly in the early weeks of treatment. The NHS medicines page on semaglutide reflects this same picture, with no mention of gout among the known reactions.

That does not mean joint pain during treatment should be dismissed. Musculoskeletal discomfort, aching limbs and general joint tenderness have been noted anecdotally and in some post-marketing reports for GLP-1 medicines as a class. The mechanism is not fully established. What it does mean is that semaglutide itself is not currently considered a direct cause of gout in the way that, say, certain diuretics or high-purine diets are.

If you have pre-existing gout and you notice a flare after starting Wegovy, it is worth telling your prescriber. Do not assume the medicine is responsible, but do not dismiss it either. The full picture matters.

Could rapid weight loss trigger a gout attack?

This is where the biology gets genuinely interesting. Gout is caused by uric acid crystallising in joints, usually the big toe, ankle or knee, when blood levels of uric acid stay too high for too long. Weight loss tends to reduce uric acid over time, which is one reason that reaching and sustaining a lower body weight is associated with fewer gout episodes in the long term. There is a catch, though.

In the short term, particularly during rapid weight loss, the body breaks down fat cells quickly, and this process can temporarily spike uric acid levels. Ketones produced during a calorie deficit can compete with uric acid for renal clearance, leaving more of it circulating. So someone who has never had gout before is unlikely to develop it solely because they are losing weight on Wegovy; someone with gout already managed at a threshold may find a flare comes slightly earlier than expected in the first weeks.

The dehydration risk that some people experience with GLP-1 medicines adds a further dimension. Reduced fluid intake, combined with nausea suppressing your appetite for drinks as well as food, can concentrate uric acid in the blood. Staying well hydrated during treatment is straightforward advice that carries real clinical weight here.

What about joint pain that is not gout, how do you tell?

Gout pain is distinctive: it tends to arrive suddenly, often overnight, affects one joint intensely (classically the base of the big toe), and the skin over the joint can appear red and feel hot to the touch. It is not the same as the generalised muscle ache some people notice after starting or increasing a dose of semaglutide, which tends to be diffuse rather than pinpointed to a single inflamed joint.

There is also a separate, confirmed signal worth knowing. The MHRA issued a Drug Safety Update in January 2026 noting that acute pancreatitis (severe, persistent abdominal pain that may radiate to the back) is a known but infrequent risk with GLP-1 medicines. That is not gout, but it is a reason to take sudden severe pain seriously and contact a doctor promptly rather than waiting. For all suspected side effects, patients can report through the MHRA's Yellow Card scheme.

Distinguishing a gout flare from other causes of joint pain generally requires a clinical assessment, sometimes a blood uric acid level, and occasionally joint fluid analysis. This is not something to self-diagnose from a list of symptoms online. If you are in real doubt, speak to a GP.

Does your history of gout affect whether Wegovy is right for you?

A past or current diagnosis of gout does not automatically rule out Wegovy. For many people with gout, the long-term benefit of sustained weight reduction on uric acid levels and on the metabolic conditions that worsen gout (insulin resistance, hypertension, high triglycerides) outweighs the small risk of an early flare. That calculation belongs to a clinician who knows your full medical history, current medications and kidney function.
