Semaglutide works primarily on appetite and gut-hormone pathways, so why muscles might hurt during treatment is worth looking at carefully — whether that discomfort comes from the medicine itself, from eating less protein, or from something else entirely. The STEP 1 trial (a 68-week study of semaglutide 2.4mg published in the New England Journal of Medicine) focused primarily on weight outcomes and the well-documented gastrointestinal side effects: nausea, vomiting, diarrhoea and constipation. Muscle pain and musculoskeletal aches were not among the frequently reported adverse events in that dataset. The NHS medicines page for semaglutide lists known common side effects, and generalised muscle pain does not appear there as a primary concern.
That said, trial data captures averages across thousands of participants. Individual experiences do not always map neatly onto published frequencies. People taking Wegovy have reported muscle aches in real-world settings, particularly in the weeks after a dose increase, and this deserves a clear explanation rather than a dismissal.
Two separate mechanisms are worth distinguishing. The first is whether semaglutide has a direct pharmacological effect on muscle tissue, the evidence for this is limited. The second is whether the lifestyle changes triggered by treatment (eating less, moving differently, losing weight quickly) create conditions in which muscles ache. That second pathway is more likely for most people, and it is something you can act on.
When appetite falls sharply (which is exactly what Wegovy is designed to do) total calorie and protein intake often falls with it. Protein is the raw material your muscles use to repair and maintain themselves. If daily protein drops well below what your body needs, muscles can become sore more easily after ordinary activity and take longer to recover. This is not a direct drug effect; it is a nutritional consequence of eating much less.
Electrolytes also matter here. Potassium, magnesium and sodium are involved in normal muscle contraction. Reduced food variety and volume, especially if nausea makes eating difficult in the early weeks of treatment, can lower intake of all three. Low magnesium in particular is a recognised cause of muscle cramps and aching, something explored in more detail on our page about muscle cramps and Wegovy.
A third factor is dehydration. GI side effects like vomiting and diarrhoea increase fluid loss. Dehydrated muscles fatigue faster and are more prone to soreness after even light exertion. Staying well hydrated during treatment is practical advice, not a disclaimer.
Finally, when weight falls quickly, the body loses some lean tissue alongside fat. This is a documented pattern with all significant calorie deficits and GLP-1 medicines are no exception. Resistance exercise and adequate protein are the two levers shown to help preserve lean mass during weight loss treatment.
Most muscle discomfort during Wegovy treatment has an explanation rooted in the changes above and responds to practical adjustments. But some muscle symptoms do need same-day or urgent medical review, and it is worth being clear about which.
Severe or sudden muscle pain, particularly if accompanied by dark or discoloured urine, extreme weakness, or fever, can indicate rhabdomyolysis, breakdown of muscle tissue that puts strain on the kidneys. This is rare and not specific to Wegovy, but it should not be sat on. Contact a doctor the same day if you notice that combination of symptoms.
Pain that is markedly one-sided, appears after a fall or sudden movement, or is accompanied by swelling and tenderness in one area may be a strain or injury rather than anything related to the medicine at all. Weight loss changes your centre of gravity and can make you more aware of your body during exercise, a good reason to build up activity gradually.
If muscle pain started shortly after a dose increase and feels more like general fatigue and achiness, it may settle over one to two weeks as your body adjusts. Keep a note of when it started, whether it is improving or worsening, and mention it at your next clinical review. A few things are consistently useful for people who notice muscle soreness on Wegovy. First, audit your protein intake honestly, most people under-eat protein when appetite is suppressed. Aiming for around 1.2–1.6g of protein per kilogram of target body weight per day is a reasonable starting point; your prescriber or a dietitian can give you a figure suited to your situation. Second, prioritise resistance activity two or three times a week. Lifting, bodyweight work and resistance bands all count. Third, keep fluid intake up even when you do not feel thirsty, this matters especially if you experience GI side effects in the early weeks.
