The phase 3 STEP trials, which supported Wegovy's UK licence, did not identify a statistically significant increase in depression or suicidal ideation in the semaglutide arms compared to placebo. That is reassuring background context. However, trial populations are screened carefully (people with active severe mental illness or recent suicidal ideation were typically excluded) so post-marketing surveillance in real-world prescribing is where the clearer picture is forming.

The European Medicines Agency and MHRA both initiated reviews of GLP-1 medicines and suicidal thoughts in 2023. The EMA concluded in early 2024 that available data did not confirm a causal link, but both agencies said monitoring would continue and that the issue was not closed.
