The most cited evidence for semaglutide 2.4mg comes from the STEP 1 trial, published in the New England Journal of Medicine, which showed an average body-weight reduction of around 15% over 68 weeks. Participants followed a reduced-calorie diet and increased their physical activity alongside the medicine, not an intermittent fasting schedule. That distinction matters. The trial does not tell us whether fasting protocols improve outcomes on top of what semaglutide already achieves.
Semaglutide works by activating GLP-1 receptors, which slow gastric emptying, reduce appetite and promote a feeling of fullness after smaller amounts of food. In practical terms, most people on Wegovy find they naturally eat less and feel satisfied sooner. The medicine is, in some sense, already doing part of what a structured fast attempts to do. Stacking a formal fasting window on top of that effect does not automatically double the benefit, and some evidence suggests the marginal gain diminishes when appetite is already suppressed pharmacologically.

Nausea is the most commonly reported side effect of semaglutide, particularly in the early weeks and after each dose step-up. The NHS semaglutide medicines page lists nausea, vomiting, diarrhoea, constipation and dizziness as typical early effects, most often settling within days to a couple of weeks.
Fasting extends the period your stomach is empty, and an empty stomach tends to make nausea from GLP-1 medicines worse rather than better. Many people on Wegovy already find they gravitate toward smaller, more frequent meals because that pattern eases GI symptoms. A long fasting window runs counter to that. Dizziness and light-headedness, which some people notice on Wegovy anyway, can also be amplified by going without food for extended periods, particularly if you are active or working through a warm afternoon without eating since breakfast.
There is also a dehydration risk worth noting. If nausea or loose stools are already present and you add a fasting period, fluid and electrolyte balance can shift more than you might expect. Staying well hydrated is always important on this treatment; it matters even more if you are restricting eating windows at the same time.

There are several distinct fasting patterns people ask about, and they do not all carry the same considerations.
Time-restricted eating (e.g. 16:8): Eating within an 8-hour window is probably the most compatible approach for someone on Wegovy, because it still allows three smaller meals if structured well and does not require full-day abstinence. Many people on semaglutide effectively arrive at something like this naturally, because the medicine reduces their appetite during morning hours. If you are already finding you are not hungry until late morning, a loose eating window may form without deliberate effort.
Full-day fasting (e.g. 5:2): Restricting calories to around 500–600 kcal on two days a week is harder to manage alongside Wegovy's GI side effects. An almost-empty stomach on fast days when nausea is already a background presence is an uncomfortable combination for many people. Whether any additional weight-loss advantage justifies that is a conversation for your prescriber, not a decision to make based on general reading.
Extended fasting (24 hours or more): There is no clinical trial evidence supporting this alongside semaglutide for weight management. The risks of dehydration, electrolyte imbalance and worsened side effects increase meaningfully. If you are considering this, speak to a clinician first.

Some people have particular reasons to be careful. If you are on Wegovy for a weight-related condition alongside a blood sugar issue, fasting can create larger glucose swings than expected. If you have a history of disordered eating, deliberately restricting food on a structured timetable can sit uncomfortably alongside treatment and deserves discussion with both your prescriber and, where appropriate, a mental health professional.
Anyone who has experienced gallbladder problems should also be aware that both rapid weight loss and fasting can increase gallstone risk independently. Semaglutide is associated with a small increase in gallbladder-related events in trials, and fasting may not be a helpful addition in that context.
People often ask about other lifestyle factors during treatment, such as whether smoking is safe to continue while taking Wegovy, and separately, whether giving blood is possible while on Wegovy, both of which are worth reviewing before making any changes to your routine.
If you are worried about weight returning on Wegovy, the answer is rarely to restrict further; it usually lies in reviewing the treatment plan with your prescriber. Fasting as a response to a plateau is not well-supported by evidence and may make side effects harder to manage exactly when you need to stay consistent.
