The Wegovy schedule starts at 0.25 mg once weekly, but this opening dose was never intended to produce significant weight loss. Its job is straightforward: give your gut a few weeks to get used to semaglutide before the dose climbs. Nausea, loose stools and stomach discomfort are more likely in the early weeks of any GLP-1 treatment, and a low starting point softens that introduction. The lowest dose of Wegovy is, in clinical terms, a preparation step rather than a treatment target.
After roughly four weeks at 0.25 mg, the standard schedule moves to 0.5 mg, then 1.0 mg, 1.7 mg, and ultimately 2.4 mg as the maintenance dose. Each step gives the body another adjustment window. The NHS's patient-level semaglutide guidance describes this graduated approach as the expected pathway. So if you are asking whether you can simply stay at 0.25 mg indefinitely because it suits you, the honest answer is that there is very little clinical evidence it will achieve meaningful weight loss at that level, and that staying there long-term would not align with how the medicine is licensed.
That said, 0.25 mg is not the only "lower" option. Any dose below the 2.4 mg maintenance level counts as a lower dose, and the question of whether someone can hold at, say, 1.0 mg or 1.7 mg is quite different from staying at the starting point.
Titration is not a conveyor belt. A prescriber can (and often does) recommend a patient spend longer at a particular dose step if side effects are difficult to manage, if there is an underlying clinical reason to be cautious, or simply if the body needs more time to adjust. This is entirely within the normal scope of how Wegovy is used, and it is one of the things a careful look at the full Wegovy dose range makes clear.
Common reasons a prescriber might extend time at a lower dose include persistent nausea that has not settled within a few weeks, poor tolerance of GI symptoms that are affecting daily life, or a clinical review that flags something worth monitoring before progressing. In those situations, staying on a lower dose is a considered medical decision, not a shortcut.

If you are asking whether you can stay on a lower dose of Wegovy long-term because you are worried about side effects or because you feel the early dose is working well enough, it is worth knowing what the trial data shows. The STEP 1 trial, published in the New England Journal of Medicine, found that adults who reached and maintained the 2.4 mg dose achieved an average weight reduction of around 15% over 68 weeks. There is no comparable long-term evidence for staying on 0.25 mg or even 1.0 mg as a permanent maintenance dose.
NICE's recommendation for Wegovy under technology appraisal TA875 specifically frames the medicine's use around the maintenance dose, and suggests reviewing whether treatment is producing enough benefit after six months on that level. If less than 5% body weight has been lost by that point, continuing may not be appropriate. Staying below the maintenance dose for the full treatment period would make that threshold harder to reach, and harder for a prescriber to justify continuing.
There is a related but distinct question: can you stay on a lower maintenance dose than 2.4 mg if that is the highest you can tolerate? Self-managing your dose (staying lower because it feels safer, or because you read about it online) is not the right approach for a prescription-only medicine. A prescriber weighs factors that a short questionnaire or a search result cannot: your weight-loss progress, your side-effect profile, any other medicines you take, and your overall health picture. Wegovy's dose schedule exists because the clinical evidence was built around it.
