The Patient Information Leaflet (PIL) and the Summary of Product Characteristics (SmPC) both state that Mounjaro KwikPens should be kept refrigerated between 2°C and 8°C. Those are also the conditions in which Mounjaro is stored and dispatched by licensed pharmacies, including ours. The documents do permit storage at or below a specified maximum room temperature for a defined number of days before first use, a practical allowance for travel, days away from home, or a fridge that isn't accessible. The NHS medicines page for tirzepatide defers to the leaflet for this figure, and so does every legitimate prescriber. That is the right approach, because the permitted period can differ between product batches and markets; printing a specific number of days here could contradict what is written inside your box. One misconception worth addressing gently: some people assume that as long as a pen looks fine (clear liquid, no cloudiness) it is safe to use however long it has been out of the fridge. Appearance alone is not a reliable test. The medicine can degrade in ways that are invisible to the eye. The time limit is the limit.
This is a separate but related question, and one worth answering clearly. Cold medicine straight from the fridge can cause more discomfort at the injection site. Many people find the injection more comfortable if the pen has been out of the fridge for about 30 minutes beforehand. That brief warm-up sits within the permitted room-temperature window and does not start a fresh clock, the allowance runs from whenever the pen was first removed from the fridge, whenever that was. If a pen has been at room temperature for longer than the leaflet permits, the guidance is straightforward: discard it. Do not inject it. Do not try to salvage the remaining doses by returning it to the fridge. The protein structure of tirzepatide, like other biological medicines, can be affected by temperature excursions in ways that are not visible and cannot be reversed by cooling the pen back down.
The same principle applies to extremes in the other direction. A pen that has been frozen (left in a car overnight in winter, for instance, or placed against the freezer compartment at the back of the fridge) should also be discarded. Equally, direct sunlight and ambient temperatures above the permitted maximum (a beach bag in summer, a car dashboard) are grounds for disposal. Used pens and discarded ones should go into a sharps container rather than a household bin. If you need one, a 1-litre sharps container is the standard size for home use with a weekly injection pen.
The permitted room-temperature window makes travel manageable. For most domestic trips or short international journeys, keeping your current pen in a cool bag (not with an ice pack touching the pen directly, that risks freezing) is a practical option. For longer trips, a small insulin-style travel cooler keeps the pen within the 2–8°C range without freezing risk.
A few things worth planning for: airlines typically allow insulin pens and similar prescription injectables in hand luggage with a pharmacy label or a letter from your prescriber. Checked luggage holds can freeze, another reason to keep the pen with you.
