The concern is understandable. Wegovy already asks a lot of your digestive system, nausea, bloating and changes to gut motility are common, especially early in treatment or after a dose step-up. Creatine, meanwhile, has a long-standing (and mostly undeserved) reputation for causing kidney stress. Put the two together and the worry becomes: is this safe?
Here is what the evidence actually shows. Creatine monohydrate is one of the most studied dietary supplements in existence. In healthy adults with normal kidney function, decades of research have found no evidence of harm at standard doses. Semaglutide is processed via peptide hydrolysis and receptor-mediated clearance, a completely separate pathway from anything creatine touches. The NHS medicines page for semaglutide lists the known interactions with Wegovy, and creatine is not among them.
The real question is not safety in the abstract. It is whether adding creatine is actually worth doing, and, if so, how to do it sensibly while your appetite and gut motility are both affected by treatment. Weight loss through any means (calorie deficit, GLP-1 medicine, or both) carries a risk of losing lean muscle tissue alongside body fat. Wegovy produces substantial weight loss; the STEP 1 trial, published in the New England Journal of Medicine, reported an average reduction of around 15% of body weight over 68 weeks. That is meaningful progress, but a proportion of it can come from lean mass unless two things are in place: adequate protein intake and regular resistance exercise.
Wegovy suppresses appetite strongly. Many people on treatment find they are eating far less than before, which means protein targets are harder to hit from food alone. Creatine does not replace protein, it enhances the output of resistance training by replenishing phosphocreatine in muscle cells, which supports strength and power. The practical effect is that it becomes easier to train hard enough to protect muscle when creatine stores are topped up. There is also a hydration point worth flagging. Creatine pulls water into muscle tissue, so total fluid needs rise. Some people also notice breathing issues on Wegovy, which can be another sign that hydration and overall physical strain during treatment deserve attention. Drinking consistently through the day, not just at mealtimes, becomes more important when you are on both.
Wegovy slows gastric emptying. That affects how quickly food (and anything dissolved in it) moves through your system. Most people tolerate creatine monohydrate well regardless, but some find that taking larger amounts in one go (as in a traditional loading phase) worsens GI symptoms they are already managing. A simple approach: skip the loading phase and take a standard 3–5g daily at a consistent time, with food and plenty of water. Many people find morning works well, before the day gets busy, before the appetite suppression from Wegovy kicks in fully.
One thing worth knowing: the tablet form of the oral Wegovy (the once-daily Wegovy tablet, approved in the UK in June 2026) has a strict morning routine, taken first thing on an empty stomach, with up to 120ml of plain water, at least 30 minutes before anything else. Taking creatine alongside Wegovy is not contraindicated by any current clinical guidance, and for someone committed to resistance training during treatment, it is a reasonable supplement to consider. The case for it is actually stronger on Wegovy than off it, given the muscle-preservation challenge that comes with rapid, appetite-mediated weight loss.
