CBD and Wegovy: what you should know before combining them

No large-scale clinical data exists on CBD combined specifically with Wegovy (semaglutide) — the absence of evidence is not evidence of safety.

Pharmacology, gastric emptying, and the practical questions worth asking your prescriber

What the pharmacology tells us about CBD and semaglutide

Semaglutide, the active ingredient in Wegovy, is a GLP-1 receptor agonist. It is not metabolised by the cytochrome P450 enzyme system in the way most small-molecule drugs are. Instead, it is broken down by the body's normal protein-digesting processes. CBD, by contrast, is processed significantly through CYP3A4 and CYP2C19 in the liver. Because those two substances travel through different metabolic routes, the classical enzyme-competition interaction that makes some drug combinations risky is less of a concern here than it would be with, say, a statin or a blood-pressure medicine alongside CBD.

That said, "less likely" is not the same as "ruled out." The NHS semaglutide medicines page advises patients on Wegovy to tell their doctor or pharmacist about every medicine, supplement or herbal product they are taking before starting treatment. That advice exists precisely because real-world combinations throw up surprises that isolated pharmacokinetic data does not always predict. A prescriber cannot assess what they do not know about.

CBD at higher doses has also been shown in some studies to inhibit CYP3A4 activity. If you happen to be taking other medicines alongside Wegovy that do rely on CYP3A4 for clearance, CBD could theoretically slow their breakdown and raise plasma levels. That is a separate concern from the Wegovy interaction itself, but worth surfacing if your medication list is longer than one item.

How Wegovy's effect on gastric emptying changes the absorption picture

One mechanism that does apply directly is gastric emptying. Wegovy slows the rate at which your stomach empties its contents into the small intestine. This is part of why it reduces appetite so effectively, but the knock-on effect is that oral substances, including CBD oil or capsules taken by mouth, may be absorbed more slowly and their peak concentration in the bloodstream may be lower or delayed compared with what you would experience without semaglutide on board.

In practical terms, this does not necessarily make CBD less effective, but it does mean the timing and dosing assumptions you built up before starting Wegovy may no longer hold. Someone who found that a particular dose of CBD oil taken at bedtime had a predictable, consistent effect may find the experience shifts after a few weeks on semaglutide. That is not a danger signal in itself, but it is information worth tracking and worth mentioning if you check in with our clinical team for a dose review.

Nausea, CBD and the GI overlap

Some people turn to CBD specifically because they have read that it may help with nausea or appetite. On Wegovy, nausea is one of the most commonly reported early side effects, usually peaking in the first few weeks after starting or after a dose increase and then easing as the body adjusts. The NHS guidance on semaglutide lists nausea, vomiting, diarrhoea and constipation as the most frequent complaints, typically mild to moderate and transient.

The idea of using CBD to take the edge off that nausea is understandable. The evidence base for CBD as an antiemetic in humans is, however, far thinner than the online conversation about it suggests. Most of the supportive data comes from animal studies or from research on nausea in chemotherapy patients, where both the context and the doses involved are very different from supplementary use alongside a weight-management medicine. Relying on CBD to manage Wegovy-related nausea instead of adjusting your meal sizes, eating slowly and reducing fatty foods is unlikely to produce the same benefit, and it sidesteps the clinical support that is actually available to you.

What to tell your prescriber, and when it matters most

The most useful step is straightforward: mention your CBD use during your consultation, or flag it to your prescriber at your next review. Note the form you take it in (oil dropped under the tongue, a capsule, a gummy), the dose in milligrams, how often you take it, and the brand if you can. That last detail matters because CBD product quality in the UK is inconsistent — some products contain significantly more or less CBD than the label states, and some contain trace THC. A prescriber cannot make a precise assessment from "I take CBD sometimes."
