One cramp pattern in the abdomen that must never be self-managed: severe, persistent pain that spreads towards the back, with or without vomiting. Some women report changes to their menstrual cycle, including more intense period cramps, after starting Wegovy. This is a distinct pattern from gut or muscle cramping and is worth understanding separately. Semaglutide itself does not directly act on the uterus, but weight loss (even modest weight loss) can shift hormonal patterns, particularly if the person had irregular cycles or PCOS before treatment. Changes in oestrogen levels as body fat decreases can temporarily alter cycle regularity and the intensity of menstruation.
There is also a contraception consideration. Women using oral contraceptives alongside Wegovy should be aware that changes to their menstrual pattern may reflect altered pill absorption, particularly during periods of significant GI disturbance.
