Dairy and Wegovy: what to know before you open the fridge

Wegovy slows gastric emptying, which can make dairy harder to tolerate for some people, not because of a drug interaction, but because of how digestion itself changes.
Lactose intolerance symptoms can surface or worsen during GLP-1 treatment, even in people who previously tolerated dairy without difficulty.
Low-fat, smaller-portion dairy is generally better tolerated than high-fat options like cream or full-fat cheese, particularly in the dose-escalation phase.
If symptoms persist, a prescriber or dietitian can help identify whether dairy is the cause and whether any dietary adjustment is needed alongside treatment.

If you've started Wegovy and suddenly find a glass of milk sitting less well than it used to, you're not imagining it. Dairy isn't banned on semaglutide — there's no clinical rule against it — but the way Wegovy slows digestion means some people notice new sensitivities, especially in the earlier weeks of treatment. This is a prescription-only medicine that a clinical prescriber assesses you for before dispensing, and your dietary questions are worth raising at that stage or with your aftercare team.

How Wegovy changes the way your body handles dairy, and what that means day to day

You pour a bowl of cereal and something doesn't feel right

It's a Tuesday morning, three weeks into your second Wegovy pen, and you've eaten porridge made with full-fat milk for years without a second thought. Now it sits heavily, and an hour later your stomach is complaining. Nothing on the box has changed. What has changed is how quickly food moves through your digestive system.

Wegovy (semaglutide) works partly by slowing gastric emptying, food leaves the stomach more gradually than before. That's a big part of why appetite reduces and people feel full sooner. The same mechanism means high-fat foods, including full-fat dairy, spend longer in contact with digestive processes that are already running more slowly. For some people that produces bloating, nausea or loose stools that weren't present before starting treatment. The NHS patient information for semaglutide lists nausea, vomiting and stomach pain among the common side effects, most of which are tied to this slowed-emptying effect. Dairy doesn't cause those effects on its own; it can amplify them if you're already sensitive.

Not everyone notices anything different. Some people continue with their usual cheese, yoghurt and milk without issue. The ones who do notice tend to do so after a dose increase, which is when gastric-emptying effects are at their strongest. If that sounds like your situation, a look at what you're actually eating and when is a reasonable first step, and the detail is worth sharing with your prescriber rather than drawing your own conclusions about whether to continue treatment.

Why something like lactose intolerance can develop or worsen on semaglutide

Lactose, the sugar in milk, needs an enzyme called lactase to digest properly. Most adults produce less lactase than they did as children, sitting somewhere on a spectrum from full tolerance to significant sensitivity. Under normal digestive conditions, that partial deficit might be invisible, your gut has time to process what arrives. When gastric emptying slows, larger amounts of lactose can reach the small intestine before being broken down, triggering the fermentation and gas production that characterise lactose intolerance symptoms.

This means a person who was managing mild, undiagnosed lactose sensitivity perfectly well before starting Wegovy might notice it clearly for the first time during treatment. It isn't an allergy, and it doesn't mean dairy has become permanently off-limits. For many people the symptoms ease as the body adjusts to the new dose. Hard cheeses (cheddar, parmesan) and strained yoghurts contain far less lactose than milk or cream, so they're often tolerated better. Smaller portions spread across the day also tend to fare better than a single large serving.

People with more complex situations (for example, those where gut symptoms overlap with other health conditions) should always discuss this with their prescriber rather than adjusting treatment independently. Our guidance on Wegovy and dairy intolerance goes into the lactose picture in more detail if that's your specific concern, and questions about gut symptoms more broadly are something our prescribers hear regularly.

Which dairy products tend to sit better, and which to approach carefully

There's no official Wegovy dietary protocol that lists foods to avoid, and you shouldn't read this as one. That said, the clinical pattern across GLP-1 medicines is fairly consistent: high-fat, high-volume foods are the most likely to cause problems, because fat delays gastric emptying further on top of the effect semaglutide is already producing.

Full-fat cream, rich cheeses eaten in large amounts and creamy sauces are the versions of dairy most likely to cause discomfort, particularly early in treatment or after a dose step. Lower-fat options (semi-skimmed milk, low-fat yoghurt, cottage cheese) are generally better tolerated. Greek yoghurt in particular is a useful food during treatment because it's high in protein, which helps protect muscle mass, and its lactose content is relatively low after straining. Protein adequacy matters on Wegovy because reduced appetite can mean reduced overall intake, so cutting out dairy entirely without a replacement source of protein and calcium isn't automatically a good idea.

If you're looking at the wider picture of how treatment affects what and how you eat, the weight-loss overview on our site covers the lifestyle context around semaglutide treatment, and our prescribers can discuss dietary considerations during your aftercare review. For a structured look at how dairy products specifically fit into the treatment picture, this page on Wegovy and dairy products is a practical companion to what you've read here.

When the symptoms are telling you something worth acting on

Mild, transient gut symptoms in the first few weeks of a new dose are common and usually settle. Symptoms that don't settle, or that are severe (particularly any strong, persistent stomach pain that radiates toward the back) need medical attention and are not something to attribute to dairy without assessment. The MHRA has highlighted acute pancreatitis as a known, infrequent but serious side effect of GLP-1 medicines, and persistent abdominal pain is the symptom to watch for; this is separate from the ordinary bloating that dairy might worsen.

More broadly, if you're unsure whether what you're experiencing is a food sensitivity, a side effect of the medicine, or something else, that uncertainty is exactly the kind of thing to raise with your prescriber rather than self-manage. Wegovy affects individuals differently, questions about how the medicine affects the skin, including how it can contribute to dryness and changes in skin texture, interactions with certain health conditions, and eye-related concerns all follow the same pattern: the detail matters, and the right answer is personal. The NHS England guidance on weight-management injections is also a useful reference for understanding what clinical oversight during treatment should look like.

If you're not yet on treatment and want to discuss how dietary factors fit into the assessment (including whether semaglutide is right for you) you can speak to our prescribers through a free consultation. There's no cost and no obligation. Payday, Monday mornings, the week before a holiday, whenever the moment feels right, the consultation is there.
