Do you feel cold on Wegovy, and should you be worried?

Feeling cold on Wegovy is not listed as a common side effect in the semaglutide Summary of Product Characteristics, but is reported by people losing weight rapidly on GLP-1 medicines.
A significant drop in body fat reduces the insulation layer under the skin, which can lower tolerance to cold temperatures.
Reduced calorie intake during treatment may also lower metabolic rate temporarily, contributing to a cooler-feeling body.
Persistent cold intolerance, or cold accompanied by fatigue, hair changes or racing pulse, warrants a conversation with your prescriber rather than self-management.

Some people do feel cold on Wegovy, and it is a recognised pattern worth understanding. Clinical trial data and post-marketing reports both show that temperature sensitivity can occur during semaglutide treatment, most often linked to the body adapting to rapid fat loss rather than any direct effect of the medicine on your thermostat. This is a prescription-only medicine assessed individually by a clinician before it is prescribed.

Why Wegovy can make you feel cold — and when it matters

What the trial data and official guidance actually record

The large STEP 1 trial, published in the New England Journal of Medicine, randomised over 1,900 adults with obesity to semaglutide 2.4mg or placebo over 68 weeks. The primary side effects captured were gastrointestinal: nausea, vomiting, diarrhoea and constipation topped the tables. Cold intolerance did not appear as a named adverse event in that dataset. The NHS semaglutide medicines page similarly focuses on GI symptoms, injection-site reactions and headache as the side effects patients are most likely to encounter.

That absence from the formal list does not mean the experience is imaginary. What it means is that cold sensitivity almost certainly arrives as a downstream consequence of weight loss itself, rather than as a pharmacological action of semaglutide on body temperature. When a medicine produces 15% average body-weight reduction over 68 weeks (as STEP 1 documented) the body changes in ways the trial was not primarily designed to capture in granular detail. Feeling cold is one of those changes, and understanding why helps decide whether it needs attention, which is something our page on whether semaglutide can make you feel cold explores in more detail.

If you want a fuller picture of how semaglutide works and what the treatment involves day to day, the semaglutide overview on this site covers the mechanism and the licensed indications clearly.

The two main reasons fat loss can leave you feeling cold

Fat tissue is not passive storage. It acts as thermal insulation, and losing a meaningful amount of it in a relatively short period genuinely reduces the body's ability to retain heat at the skin surface. People who lose weight quickly by any method, not just GLP-1 medicines, frequently notice this. The effect can be particularly noticeable in the hands and feet, and in rooms or seasons that previously felt comfortable.

The second factor is calorie reduction. Wegovy works in part by suppressing appetite, and most people on treatment eat considerably less than before. Metabolism is partly a function of how much energy is coming in; a sustained calorie deficit can nudge basal metabolic rate downward, producing a mild but perceptible feeling of running cooler, and our guide to feeling cold on Wegovy walks through exactly why this happens and what to expect. This is a normal physiological response, not a sign that something has gone wrong.

For a broader look at how people feel across the different phases of treatment, the page on how you feel on Wegovy covers the range of experiences clinicians hear about most often.

When feeling cold deserves a closer look

Cold that settles in gradually as weight drops is generally explainable. Cold that arrives suddenly, or comes packaged with hair thinning, unexplained fatigue, constipation unrelated to GLP-1 dosing, or a heart rate that feels either unusually fast or slow, points toward thyroid function and warrants a blood test. Hypothyroidism can develop independently of weight-loss treatment, and the symptoms overlap enough with GLP-1 side effects that they are easy to attribute to the wrong cause.

The same applies if you feel cold alongside persistent dizziness or very low energy, and our dedicated page on whether Wegovy makes you feel cold covers the signs that suggest a chat with your prescriber is worthwhile. Significant calorie restriction can, in some people, tip into nutritional insufficiency (particularly for protein, iron or B12) any of which can amplify cold sensitivity. Your prescriber is the right person to work through this, not a supplement shelf.

Suspected side effects from any prescription medicine can be reported directly to the MHRA via the Yellow Card scheme — it is open to patients as well as healthcare professionals, and reports from people on treatment genuinely shape future safety guidance.

If you are already in treatment and want to talk through what you are experiencing, reaching out to the team is what the aftercare service is there for.

Practical steps if cold sensitivity is affecting daily life

The most effective practical measures are the predictable ones: layering clothing, prioritising warm meals and hot drinks, and keeping moving, light resistance or walking keeps peripheral circulation active. Protein intake deserves particular attention on treatment; adequate protein helps preserve muscle mass, which is more metabolically active than fat and generates more body heat at rest.

Cold sensitivity tends to plateau rather than worsen indefinitely. As the body adapts to its new weight, the perception of cold usually stabilises. People who have gone through the early phases of rapid loss often report that the chill they noticed at three or four months had largely resolved by the time their weight settled.

It is worth checking whether the medicine itself is being stored correctly. Wegovy pens are refrigerated at 2–8°C until use; the exact in-use window before a pen should be discarded is set out in the Patient Information Leaflet inside the box. When your order arrives (tracked by DPD, in plain unbranded packaging) the leaflet is included and is the definitive reference for storage and handling.

If you are considering Wegovy for the first time, the Wegovy treatment page sets out the licensed indications, and the weight-loss overview covers how the different options compare. Treatment is available only following a clinical assessment; a prescriber reviews every consultation before anything is issued.
