The first pen of Wegovy contains 0.25 mg semaglutide. This starting dose exists specifically to give your system time to adapt, it is not meant to suppress appetite strongly or shift the number on the scale. Think of it as the settling-in period.
That does not mean nothing is happening. Semaglutide slows the rate at which food leaves your stomach, and it acts on areas of the brain involved in hunger signalling. Some people report a quieter appetite even at this stage. The most common experience in week one is mild nausea, particularly after eating. This is the side effect people hear about most, and for good reason, it affects a meaningful proportion of users early on. It usually improves within days to a couple of weeks. Eating smaller portions, avoiding greasy food and staying well hydrated all help. After four weeks at 0.25 mg, the prescriber's titration schedule moves the dose up, first to 0.5 mg, then to 1.0 mg, continuing roughly every four weeks toward the 2.4 mg maintenance dose. Each step can bring a more noticeable shift in appetite.
This is the phase where many people describe a genuine change in their relationship with food. Meals that used to feel easy to finish become harder to get through. Cravings for high-calorie foods can quieten. The urge to snack between meals fades for some people almost completely.
A practical checking habit worth building: before each meal during this phase, pause for thirty seconds and honestly rate your hunger on a scale of one to ten. People often find, after a few weeks, that they are starting meals at a four or five rather than an eight, and that small shift adds up over months. Weight loss during this middle phase is usually gradual, half a kilogram to a kilogram per week on a reduced-calorie diet is a realistic range for many, though results vary considerably. The STEP 1 trial, published in the New England Journal of Medicine, followed participants for 68 weeks and recorded around 15% average body-weight reduction at the 2.4 mg maintenance dose. That trajectory is a marathon, not a sprint.
Once someone reaches and stabilises at the 2.4 mg weekly dose, appetite suppression tends to be at its most consistent. Many people at this stage describe food noise (the background mental chatter about eating) as significantly quieter than before treatment.
It is worth knowing that the effect can plateau. If weight loss slows or stops, that is a signal to review the whole picture: calorie intake, protein adequacy, activity levels, sleep, stress. A newer 7.2 mg maintenance dose has also been approved by the MHRA (the dedicated single-dose pen was authorised on 14 April 2026) for adults with a BMI of 30 or above. Trial data at this higher dose reported around 20.7% average weight loss over 72 weeks, narrowing the gap with tirzepatide results. Whether this dose is appropriate is always a clinical decision made with your prescriber.
