Hair loss is a recognised side effect of Wegovy (semaglutide), though it affects a minority of people and is almost always temporary. What the evidence actually says about hair and semaglutide, and when to take it seriously

Is hair loss a direct effect of semaglutide, or is something else going on?

This is the question our prescribers hear regularly, and the answer is more nuanced than a simple yes or no. Semaglutide does appear in clinical-trial adverse-event data as a cause of hair loss, but the mechanism points strongly to the caloric deficit and rapid weight change rather than the molecule itself. When the body loses weight quickly, a significant proportion of hair follicles can shift prematurely from the active growth phase (anagen) into the resting phase (telogen), then shed together two to four months later. This is telogen effluvium.

The timing catches people off guard. You start Wegovy, feel it working, lose weight through the first few months, and then, just when things seem to be going well, the shedding begins. That delay between the trigger and the visible effect is entirely typical of telogen effluvium and has nothing to do with the medicine becoming harmful. It is the body processing earlier stress.

Protein intake is particularly relevant here. Appetite suppression from semaglutide can reduce overall calories substantially; if protein falls below what hair follicles need, shedding accelerates. How common is hair loss on Wegovy, and how long does it last?

In the STEP 1 trial, the pivotal 68-week study of semaglutide 2.4mg published in the New England Journal of Medicine, alopecia was recorded in a small proportion of participants, higher than in the placebo group but still a minority experience. The NHS classifies it as an uncommon side effect of semaglutide, meaning it occurs in fewer than one in ten people.

Duration varies. For most people, shedding slows and eventually stops once weight loss stabilises and nutrition catches up. Full regrowth can take several months after shedding stops, the hair cycle moves slowly. A smaller number of people find the shedding continues while they are still losing weight actively, then resolves in the maintenance phase.

One practical note: if your first pen arrived just before a bank holiday weekend, or you ordered mid-month when payday landed, and you noticed shedding around three months later, the timing follows exactly the pattern described above. Telogen effluvium does not respect calendars.

When should you actually get medical help?

Not all hair loss during Wegovy treatment is telogen effluvium. There are situations where you should contact a healthcare professional promptly rather than waiting it out.

Contact your GP or prescriber if the shedding is patchy rather than diffuse (scattered bald spots may suggest alopecia areata, an autoimmune process unrelated to weight loss), if it is accompanied by other symptoms such as fatigue, cold intolerance or significant changes in mood (thyroid function changes are worth ruling out), or if blood tests have not been done recently and you have been on a heavily restricted diet.

Iron deficiency is common during weight loss and is one of the most reversible causes of hair thinning; a simple blood panel can identify it. Ferritin, full blood count, thyroid function and vitamin B12 are all reasonable baseline checks if shedding has been ongoing for more than three months.

Reports from patients contribute directly to post-market surveillance. What can you actually do about it?

There is no single intervention that stops telogen effluvium in its tracks, but several things make a real difference to recovery speed and severity.

Protein intake is the biggest lever most people have. Aim for a meaningful portion of protein at every meal: hair follicles are essentially made of keratin, a protein, and they lose out when overall dietary protein drops. A registered dietitian can help if you find appetite suppression is making it genuinely difficult to eat enough.

Treating any identified deficiencies (iron, B12, vitamin D) with appropriate supplementation is also worthwhile, but supplementing without a deficiency is unlikely to help and some supplements interact with other medicines, so check with a prescriber first.

Gentle hair care helps minimally but practically: heat styling, very tight hairstyles and harsh chemical treatments can worsen mechanically fragile growing hairs. Beyond that, the evidence for specific hair-supplement products is weak, and if you are also noticing unusual digestive symptoms such as sulphur burps while taking Wegovy, it is worth mentioning those to your prescriber at the same time.
