There's a gap between what the medicine's licence allows and what the NHS will fund, and it matters here. NICE's technology appraisal for Wegovy (TA875) recommends it only within specialist weight management services, with a maximum treatment duration of two years, and with at least one weight-related comorbidity. The NHS BMI thresholds used in practice sit at 35 and above for most people, with 32.5 used for the same ethnic-background groups. Waiting lists for those services tend to be long.

Private prescribing follows the product licence rather than NICE's commissioning criteria. That means a BMI of 27–29.9 with a qualifying condition is genuinely on the table, not a theoretical footnote. What happens if you meet the BMI criteria but have a complicating factor

Eligibility isn't simply a box-ticking exercise. Some medical histories mean Wegovy isn't suitable regardless of BMI. A history of medullary thyroid carcinoma or multiple endocrine neoplasia type 2 rules it out. Active pancreatitis or a history of serious pancreatitis requires careful prescriber consideration. Pregnancy, breastfeeding, or planning to conceive in the near term also means it isn't recommended, and anyone using oral contraceptive pills should know that semaglutide's effect on gut motility could affect absorption, so discussing contraception with your prescriber is part of the process.

Age matters too. Wegovy is licensed for adults only; the minimum age requirement is 18. Certain gastrointestinal conditions also warrant a detailed conversation before starting.

These aren't reasons to assume you won't be suitable, most people with a BMI in the eligible range and one qualifying condition will not have disqualifying factors.
