Does Mounjaro Have to Be Kept in the Fridge?

You've just taken your pen out of the fridge, now what?

Picture the scene: it's Saturday morning, you've got your injection ready, and you realise you left your Mounjaro pen on the kitchen counter overnight. You're unsure whether it's still usable. This is one of the questions our prescribers hear most weeks, and the short answer is: check the leaflet first, because the permitted room-temperature window is stated there precisely.

What we can say generally is that Eli Lilly has built some out-of-fridge tolerance into the product, but the exact number of days allowed at room temperature appears in your pen's Patient Information Leaflet and in the Mounjaro SmPC on the eMC. If you are unsure whether your pen has stayed within that window, contact our aftercare team or a pharmacist before injecting. A pen that is out of condition looks identical to a usable one (there is no discolouration or obvious sign) so the only safe guide is the time and temperature record.

Temperature aside, light matters too. Keep the pen in its original carton, which protects it from light even when it is sitting in the fridge door. Think of the carton as part of the storage system, not just packaging to discard. Fridges, travel and the practicalities most people don't think about until they need them

Most storage problems happen not at home but in transit. A weekend away, a work trip, a holiday, suddenly you are thinking about airline rules for medication in hold luggage (do not put it there; cold holds can freeze the pen) and hotel mini-fridges that run colder than they should.

The rules are simple in principle. Keep the pen refrigerated. Use an insulated cool bag with an ice pack for journeys, but keep the pen away from direct contact with the ice, which could freeze it. Frozen Mounjaro must be discarded, it cannot be thawed and used. Once you arrive, check the fridge temperature if you can; anything below 2°C is too cold, anything above 8°C is outside the licensed storage range.

At home, the fridge door is fine as long as temperatures stay between 2 and 8°C. Many standard UK fridge doors sit in that band. What you want to avoid is the drawer nearest the ice compartment in a combined fridge-freezer, which can dip below zero. If you are mid-treatment and thinking about what storage looks like month to month, it is worth knowing that each pen holds four individual doses. You keep the same pen in the fridge for the whole month, removing it only on injection day. That is not four trips to a pharmacy, it is one delivery, one pen, used weekly.

What freezing, heat or light exposure means for your dose

Tirzepatide, the active medicine in Mounjaro, is a protein-based molecule. Proteins are sensitive to extremes of temperature and to light in a way that small-molecule drugs often are not. Freezing can denature the molecule (change its structure so it no longer works as intended) and there is no reliable way to detect that damage by eye. The liquid will look the same. Heat exposure above 30°C, or prolonged exposure above 25°C, risks similar degradation, though the specific thresholds and permitted durations are again those stated in your leaflet.
