What the STEP trials and regulators actually say about semaglutide and blood

The STEP 1 trial (a 68-week randomised controlled study published in the New England Journal of Medicine) enrolled more than 1,900 adults with obesity and tracked a wide range of safety outcomes alongside weight loss. Blood-thinning effects were not among the adverse events identified. The trial found the expected profile for a GLP-1 medicine: gastrointestinal symptoms (nausea, diarrhoea, constipation) were the dominant side effects, reported in roughly three-quarters of the active-treatment group at some point during the study. These were generally mild to moderate and most pronounced in the first weeks.

Some people notice they bruise a little more easily after losing weight rapidly, because subcutaneous fat that once cushioned small knocks is reduced. What actually shows up in blood tests when you are on semaglutide

Semaglutide itself does not appear on a standard blood panel, it is not a substance that routine tests are designed to detect. What clinicians do see over time on treatment is the downstream effect of weight loss: improvements in HbA1c, fasting glucose, cholesterol fractions, liver enzyme readings and inflammatory markers all trend positively in most patients. These are secondary to the weight reduction rather than a direct pharmacological action of the medicine on blood composition.

There are some findings worth knowing. Semaglutide is associated with a modest heart rate increase in some individuals, which is monitored in clinical studies and noted in the prescribing information. It is not linked to haematological changes (changes to red cells, white cells or platelets).
