The STEP 1 trial (the pivotal 68-week study published in the New England Journal of Medicine that supported Wegovy's licence) recorded gastrointestinal events as the most common adverse effects in participants taking semaglutide 2.4mg. Nausea, vomiting, diarrhoea and constipation drew most of the attention, but indigestion and acid reflux featured too. The NHS semaglutide medicines page lists heartburn and indigestion explicitly among common side effects, meaning they occurred in more than one in every hundred people in clinical data.
The underlying reason is straightforward. Semaglutide activates GLP-1 receptors that, among other things, slow gastric motility. Food sits in the stomach longer, and that extended dwell time increases the opportunity for stomach acid to travel back into the oesophagus. Reflux isn't a sign the medicine is failing — it is often a sign it is working exactly as intended, just in a way your oesophagus finds uncomfortable.

A common assumption is that heartburn on Wegovy means the dose is too high and needs reducing. That's worth questioning. Reflux and indigestion frequently appear at the 0.25mg starting level and improve on their own before the next titration step. Dose-stepping exists partly to give your system time to adapt; it doesn't mean the body treats each increase as a fresh insult on the same scale.
That said, timing does matter. Symptoms are often worse when people eat large meals, eat quickly, or lie down within an hour or two of finishing a plate. On Wegovy, the stomach takes longer to clear, so the usual post-meal window before reclining becomes more important. Evening meals eaten late and followed immediately by bed are a reliable trigger for people prone to reflux even without semaglutide, the medicine sharpens that sensitivity.
Fatty, spicy and acidic foods tend to compound the problem. Carbonated drinks add gas, which increases pressure on the lower oesophageal sphincter. Caffeine relaxes it. None of this is unique to Wegovy, but the slowed emptying amplifies effects that might otherwise be barely noticed. Smaller, more frequent meals (and giving yourself at least 90 minutes upright after eating before lying down) are the two practical changes that make the most difference. Mild reflux that eases after a few weeks usually doesn't need intervention beyond the dietary adjustments above. But there are situations where symptoms need prompt clinical attention rather than patient waiting.
Severe, persistent pain in the upper abdomen (particularly pain that radiates toward the back) should never be attributed to ordinary indigestion and left alone. The MHRA highlighted acute pancreatitis as an infrequent but serious known risk with GLP-1 medicines in a Drug Safety Update issued in January 2026; the symptom overlap with bad reflux is real, and only a clinician can tell them apart. Similarly, difficulty swallowing, vomiting blood, or reflux that isn't responding to anything after several weeks warrants a review.
Over-the-counter antacids are used by some people taking Wegovy for short-term relief, but whether they're appropriate for you (and whether any specific product interacts with your other medicines) is a question for your prescriber or pharmacist, not a search engine. The timing of any oral medicine can also matter when gastric emptying is slowed; your prescriber is best placed to advise on that too.
