The Wegovy Summary of Product Characteristics, published on the electronic Medicines Compendium, lists its side-effect profile in detail. Nausea, vomiting, diarrhoea, constipation, indigestion and fatigue feature prominently, these are the effects that dominate the early weeks of treatment. Skin dryness as a standalone adverse reaction does not sit in the common or very common categories. That matters, because the SmPC draws on the large clinical trial programme and post-marketing surveillance; effects seen frequently enough to be attributable to the medicine are included. The NHS medicines page for semaglutide echoes this, listing the GI effects as the ones most people experience, particularly after starting treatment or a dose increase.

That does not mean no one experiences dry skin. Why dehydration is the more likely explanation

The mechanism here is straightforward. Wegovy slows gastric emptying and suppresses appetite via GLP-1 receptor activation. In the early weeks, nausea is common enough that many people eat and drink noticeably less. If vomiting or diarrhoea also occur, fluid losses accelerate. Skin is one of the first organs to reflect even mild dehydration, it loses elasticity, feels tight and may look dull or flaky. This is a hydration effect, not a direct dermatological one.

A simple check: press gently on the skin on the back of your hand for a couple of seconds, then release. Healthy hydrated skin springs back almost instantly. If it takes a beat longer to flatten, that sluggishness can indicate you need more fluid. It takes under a minute and costs nothing. Aiming for consistent water intake throughout the day, especially during weeks when nausea is active, addresses the root cause rather than just the symptom. Distinguishing dryness from itch, rash or an allergic response

Dry, slightly tight skin in the early weeks of treatment is different from persistent itch, spreading redness or a raised rash. Itching on Wegovy can have several origins: very dry skin is one; an injection-site reaction is another (mild localised redness or itching at the injection spot is documented); and, less commonly, a broader hypersensitivity reaction. The STEP 1 phase 3 trial (1,961 adults with obesity, 68 weeks, published in the New England Journal of Medicine) captured adverse events rigorously, and serious allergic reactions were rare but did occur, consistent with all injectable medicines.

If itching is mainly at the injection site and resolves within a day or two, rotating sites and ensuring the pen has been at room temperature for a moment before injection can help. If itching is widespread, if there is swelling around the face or throat, or if breathing changes, that is an emergency.
