This is the right question to ask, and it deserves a straight answer. The summary is: the risk exists and is taken seriously by regulators, but for most people it is not the dominant experience on treatment. Wegovy's prescribing information lists depression and mood changes as potential side effects, and the NHS advises patients to watch for them — you can read the full guidance on the NHS semaglutide medicines page. The MHRA keeps post-market safety data under ongoing review for all GLP-1 medicines, which is why Wegovy still carries a Black Triangle (▼), meaning regulators continue to collect reports.
What the large trials did not show is a statistically clear causal relationship between semaglutide and clinical depression at the population level. The STEP 1 trial, published in the New England Journal of Medicine, monitored mental-health outcomes as part of its safety data and did not find a signal. That does not mean no individual experiences low mood, it means, on current evidence, most do not. Individual responses vary, and there are plausible biological reasons why both directions are possible, which the next two sections cover.
If you have a history of depression or another serious mental-health condition, that is not a flat barrier to treatment, but it is exactly the kind of detail a prescriber needs to know about before writing a prescription. It shapes the conversation, not the conclusion.
Semaglutide slows gastric emptying and acts on GLP-1 receptors that exist not just in the gut but in the brain. The central effects are part of why appetite falls, the brain receives satiety signals differently. Those same pathways may, in some people, influence mood, motivation, or the reward response to food. For people whose relationship with eating was emotionally significant, losing that pleasure abruptly can feel disorienting. A few patients describe a kind of flatness: less enjoyment of meals, less social spontaneity around food, and sometimes a generalised emotional blunting. That is a real experience worth acknowledging.
On the other side, many people report an unexpected lift in mood within the first weeks. Possible reasons include: reduced blood-sugar fluctuations (which can themselves cause low mood and irritability), improved sleep as weight comes off, and simply the psychological relief of seeing progress after years of difficulty. How Wegovy makes you feel overall is a genuinely mixed picture, shaped heavily by your starting point, your mental-health history, and what else is happening in your life. There is also a body of early research exploring GLP-1 receptors in mood regulation, but it is not yet at a stage where clinical conclusions can be drawn.
Other physical side effects can compound emotional strain indirectly. Persistent nausea is demoralising. Fatigue in the early weeks can look like low mood. If you feel weak or unusually tired alongside a low emotional state, that is a conversation for your prescriber, fatigue and weakness on Wegovy are covered separately.
The NHS and MHRA are clear: if you develop depression, notice a significant change in mood, or have any thoughts of self-harm or suicide while taking Wegovy, tell a healthcare professional straight away. Do not wait to see whether it passes. Stop the injection and seek urgent help if you feel you might harm yourself. This is not a theoretical precaution, it reflects the same standard applied to many medicines that act on neurological pathways.
For less acute concerns, the right first step is your prescriber or GP.
