Extreme Tiredness on Wegovy: Should You Push Through or Act?

Fatigue and tiredness are listed as recognised side effects of semaglutide (Wegovy), most commonly reported when treatment begins or after a dose step-up.
The mechanism is partly linked to the GI side effects themselves: nausea, reduced appetite and lower calorie intake can all sap energy, especially in the early weeks.
Severe, persistent stomach pain radiating to the back is not tiredness — it is a signal to seek urgent medical help; the MHRA highlighted acute pancreatitis as a known serious risk with GLP-1 medicines in its January 2026 Drug Safety Update.
If fatigue is affecting your safety or daily function and does not ease as your body adjusts, contact your prescriber, dose timing, hydration and protein intake are practical things a clinician can review.

Fatigue is one of the more common complaints in the first weeks on Wegovy, and for some people it tips beyond mild tiredness into something that genuinely disrupts daily life. The question most people face is straightforward: is this normal, does it pass, and is there anything you should actually do about it? The short answer is that low-level tiredness is a recognised side effect, most noticeable when treatment starts or a dose increases, and it typically settles within days to a couple of weeks — but the full picture of Wegovy's side-effect profile matters here, because a handful of signs warrant prompt medical attention rather than patience. These are prescription-only medicines; any persistent or severe symptom is worth raising with your prescriber rather than waiting it out alone.

Making sense of fatigue on Wegovy: what's expected, what's manageable, and what needs a call

Is what you're feeling within the normal range?

Wegovy works in part by slowing gastric emptying and reducing appetite sharply, particularly early in treatment. Eating significantly less than usual, and digesting what you do eat more slowly, means your body is taking in fewer calories than it was used to. That alone can produce a heavy, low-energy feeling, not dramatic exhaustion, but the kind of tiredness that makes a late afternoon feel longer than it should.

On top of that, nausea and disrupted eating patterns mean some people are inadvertently skimping on protein and fluids, which compounds things. The NHS medicines guidance on semaglutide lists fatigue, headache and dizziness alongside the more commonly cited GI symptoms, so if this is what you are experiencing, you are not imagining it and it is not a signal that something has gone wrong with your prescription.

The pattern to expect: tiredness is typically most noticeable in the first one to two weeks after starting, or in the days following a dose increase, then gradually settles. If yours fits that arc and is manageable (you can still get through a working day, you are not feeling faint) watchful waiting with small practical adjustments is reasonable. Our guide to Wegovy tiredness covers what to expect at each stage of treatment, and eating adequate protein at each meal, staying properly hydrated and not cutting calories more aggressively than the medicine is already doing for you can all make a tangible difference within a few days.

When the tiredness is telling you something else

The decision shifts when fatigue comes alongside other symptoms, particularly GI ones. Persistent vomiting or diarrhoea causes dehydration, and dehydration causes fatigue, the two feed each other quickly. If you cannot keep fluids down for more than a day, or if you feel dizzy when you stand, that is dehydration needing attention, not standard tiredness to ride out.

A more urgent picture: severe stomach pain, especially if it is persistent and radiates toward the back, with or without vomiting, is not typical GI discomfort. In January 2026, the MHRA issued a Drug Safety Update for GLP-1 receptor agonist medicines specifically flagging acute pancreatitis as a known but infrequent serious risk. The tiredness that comes with pancreatitis is not the low-level fatigue of early treatment, it sits alongside that characteristic abdominal pain. Do not wait to see if it resolves. Seek urgent medical help.

Other symptoms worth reporting promptly include signs of an allergic reaction, sudden mood changes or low mood, and any yellowing of the skin or eyes, which could point to gallbladder involvement. These are not common, but they are the reason the guidance exists.

Practical decisions you can make right now

For most people dealing with ordinary, manageable tiredness in the first fortnight, a few concrete adjustments tend to help. Aim for 25–30g of protein at each meal even when appetite is low, it is easier than it sounds if you lead with eggs, Greek yoghurt, chicken or fish before anything else on the plate. Keep a water bottle somewhere you will see it; the tablet-before-the-kettle-goes-on routine works for the same reason, habit placement, not willpower.

Avoid pushing yourself hard in the gym during the adjustment phase. Gentle walking is fine; high-intensity sessions that further deplete glycogen stores when your intake is already lower than usual can leave you floored. Give it two weeks at a new dose before deciding the tiredness is permanent.

If you are reviewing the overall costs and practicalities of private treatment, the Wegovy price comparison page covers what the UK market looks like and what a legitimate provider's pricing typically includes. More detail on living with the adjustment period, including hair changes some people notice, is in our guide on managing Wegovy side effects over time.

What your prescriber can actually do about it

Tiredness that does not settle, or that feels disproportionate to what you would expect from a lower-calorie period, is worth raising. Your prescriber has several levers: reviewing whether your injection timing could be adjusted, checking whether the dose step-up schedule suits your physiology, and advising on whether any other medicines you take could be interacting in a way that compounds fatigue.

This is not a minor concern to apologise for mentioning. A GPhC-registered Independent Prescriber (the kind who reviews every consultation at our pharmacy) is there specifically to make these clinical judgements, and doing so is part of the ongoing care that legitimate treatment should include, not an extra. If you are on Wegovy through another provider and finding aftercare difficult to access, that is worth factoring into any future decision about where you manage your treatment.

You can also report any suspected side effects directly via the MHRA's Yellow Card scheme, which exists precisely so regulators can track patterns across the population of people taking these medicines. It is straightforward and anonymous if preferred. For more on why Wegovy causes tiredness in some people and not others, or the broader question of semaglutide and fatigue mechanisms, those pages go further into the physiology.
