What the trial data actually showed about semaglutide for weight loss

The evidence base for semaglutide as a weight-loss medicine begins with STEP 1, a 68-week trial published in the New England Journal of Medicine that enrolled adults with obesity but without type 2 diabetes. Participants receiving semaglutide 2.4mg alongside lifestyle changes lost an average of around 15% of body weight, compared with roughly 2.4% for placebo. That trial shaped the MHRA's original Wegovy licence and, later, NICE's appraisal of semaglutide for weight management (TA875), which recommends it within specialist services for adults with a BMI of 35 or above and at least one weight-related condition, for a maximum of two years.

Since then the dose ceiling has risen. The MHRA approved a 7.2mg maintenance dose in January 2026, and on 14 April 2026 approved a dedicated single-dose 7.2mg pen, one weekly injection with a built-in covered needle that locks after use. Trials at this higher dose reported average weight loss of around 20.7% over 72 weeks, narrowing the gap with tirzepatide. The pen format and dose availability for any individual patient is confirmed during clinical assessment, not assumed in advance.

In June 2026 the picture widened further when the MHRA approved the first oral semaglutide tablet licensed for weight loss in the UK, branded as Wegovy tablets. The phase 3 OASIS 4 trial (307 adults, 64 weeks) reported average weight loss of around 13.6% versus roughly 2.4% for placebo; among participants who adhered fully to treatment that figure was closer to 17%. No refrigeration is needed, a meaningful practical difference from the injection for some people.

Same generic name, three different licences: how Wegovy, Ozempic and Rybelsus differ

The generic name for Ozempic and Wegovy is indeed the same (semaglutide) but the products are not interchangeable under UK law. Ozempic (0.5mg, 1mg, 2mg injection) is licensed only for type 2 diabetes. The effects include slowing gastric emptying, reducing appetite signals reaching the brain, and supporting better blood-sugar regulation.
