Start journey Learn moreThe 0.25mg dose of Wegovy is not designed to produce significant weight loss. It is a tolerability dose — its job is to help your body adjust to semaglutide before the real treatment begins. Most people see little change on the scales at this stage, and that is entirely expected. Here is what the starting dose actually does, how long it lasts, and when meaningful results tend to follow.

Semaglutide works by activating GLP-1 receptors in the gut and brain, slowing digestion and reducing appetite. That mechanism is powerful, and introducing it too quickly leads to nausea, vomiting and stomach cramps that are uncomfortable enough to make people stop treatment altogether.
The 0.25mg starting dose is the answer to that problem. It is set low enough that most people barely notice any gastrointestinal effect, giving the body four weeks to become accustomed to the medicine before the dose steps upward. The NHS medicines page for semaglutide describes it plainly: 0.25mg is a starting dose and is not intended as a weight-loss dose.
So if you are currently on 0.25mg and wondering whether semaglutide is doing anything useful at this early stage, it is. Just not the part you are waiting for yet.
The licensed UK titration runs 0.25mg for four weeks, then 0.5mg, 1.0mg, 1.7mg, and finally 2.4mg, each level held for roughly a month before progressing. Your prescriber decides when a step up is appropriate; if side effects are still noticeable at 0.25mg, staying there a little longer is sometimes the right call.
A practical habit worth building early: before each new pen, note your current weight. A quick note on your phone takes under a minute and gives you and your prescriber a clear picture of your trajectory as doses increase. It removes a lot of guesswork at review.
For a fuller picture of the whole titration journey, the Wegovy doses guide walks through each step and what patients typically experience at each level. People who want to understand how effective the 0.5mg dose is once they move on from the starter phase often find that context useful too.
The evidence behind weight loss at maintenance dose comes from the STEP 1 trial, 68 weeks of treatment at 2.4mg produced an average body-weight reduction of around 15%, as published in the New England Journal of Medicine. The 0.25mg phase is the foundation that makes reaching that dose possible for most people.
Patients sometimes ask whether staying on 0.25mg indefinitely would be safer or gentler in the long run. The short answer is no, at that dose, the appetite-suppressing effect is minimal, and the medicine's benefits for weight management are not meaningfully activated. The titration is not just a convention; it reflects how the pharmacology works.
By the time most people reach 1.7mg or 2.4mg, appetite reduction tends to become noticeably more pronounced. That is also when the most clinically significant results are typically seen. Progress before that point can feel slow, which is worth knowing in advance.
Wegovy is licensed in the UK for adults with a BMI of 30 or above, or 27 or above alongside a weight-related health condition. Lower thresholds can apply for some ethnic backgrounds under UK guidance. The broader Wegovy overview covers eligibility in more detail, alongside what the full treatment course looks like in practice.
If you are thinking about cost as you plan ahead, the Wegovy pricing page sets out what private treatment typically costs in the UK and what a legitimate price should include.
Side effects on 0.25mg are usually mild, some mild nausea, perhaps a little less appetite than usual, occasionally loose stools or constipation. These tend to settle within a week or two. If they are persistent or more severe, that is worth raising before any dose increase rather than pushing through.
What should prompt earlier contact: severe stomach pain, especially pain that radiates to the back, or signs of dehydration from repeated vomiting. The MHRA has highlighted acute pancreatitis as a known but infrequent risk with GLP-1 medicines, the Yellow Card scheme is where suspected side effects can be reported, and our team is available seven days a week for aftercare questions.
Wegovy is a prescription-only medicine. A prescriber assesses your full history before treatment starts and reviews your progress at each stage, not an automated system.
