Start journey Learn moreMost people starting Wegovy lose weight gradually over several months, not in dramatic early drops. In the pivotal STEP 1 trial, participants lost around 15% of their body weight on average over 68 weeks — roughly a steady 1–2 lb per week once the medicine is working at a therapeutic dose. That pace is deliberate, and it matters. Wegovy is a prescription-only medicine: a GPhC-registered prescriber assesses whether it is clinically suitable for you before any treatment begins.

The first four to eight weeks on Wegovy are often quieter than people expect. At 0.25mg, the starting dose, the medicine's job is to let your body adjust, nausea is the most common early side effect, and a slow build reduces it. Appetite suppression at this stage is subtle for many people, and measurable weight change can be modest. Some people notice nothing on the scales at all for the first month.
That is completely normal. The dose steps up roughly every four weeks, overseen by your prescriber, and meaningful appetite reduction tends to follow the later increases. Comparing week two of a 0.25mg starter against someone else's week twenty at 2.4mg is a recipe for unnecessary worry. How long Wegovy takes to work depends heavily on which dose you are currently on, not just how many days have passed since your first injection.
One practical note worth mentioning: if your first pen arrives in the middle of a hectic week, over a bank holiday, or the day before a big family meal, don't read anything into that first fortnight. Timing is rarely perfect. What matters is consistency once you are settled at a dose.
The STEP 1 trial, published in the New England Journal of Medicine, followed adults using semaglutide 2.4mg weekly alongside lifestyle changes for 68 weeks. The average total weight reduction was around 15% of starting body weight. Spread across the trial period, that works out to roughly 0.5–1kg per week during the most active phase, slowing as participants approached their plateau.
A few things shape your personal rate. People with higher starting weights often see larger absolute losses early on, even if the percentage is similar. How closely you follow the dietary and activity guidance your prescriber recommends plays a real role. Stress, sleep, and underlying health conditions all affect the pace too. So does the dose you have reached, at 1.7mg versus 2.4mg, the effect on appetite differs noticeably for many people.
The factors that influence how fast you lose weight on Wegovy are more varied than most people realise going in. Aiming for a steady 1–2 lb per week at therapeutic doses is a reasonable expectation; aiming for faster is not something the prescribing guidelines support, and it is not how the medicine works.
Almost everyone on Wegovy hits a period where the scale stops moving. This can last weeks, occasionally longer. It is one of the most common concerns our prescribers hear, and it is also one of the most misunderstood.
Your body adapts to a lower calorie intake by reducing its metabolic rate, a well-documented response to sustained weight loss. Wegovy continues to work on appetite during a plateau; the visible result just pauses while your physiology recalibrates. Protein intake, strength exercise, and hydration all help during this phase. A weight-loss stall on Wegovy does not automatically mean the treatment is failing or that the dose needs changing, that judgement belongs to your prescriber.
NICE guidance, published in its appraisal of semaglutide for weight management (TA875), recommends reviewing whether treatment is still appropriate if someone has not lost at least 5% of their body weight after six months at the maintenance dose. If you are approaching that threshold and concerned, the right conversation is with your prescriber, not a dose adjustment made on your own initiative.
First, check the comparison you are making. Social media is full of people sharing unusually fast results, those posts are not representative of average experience in a clinical trial, and trial participants are not a random sample of the general population either. The question to ask is not "am I losing as fast as that person online?" but "am I making meaningful progress in the right direction over the right timeframe?"
Second, review what is happening alongside the injections. Wegovy works best with a reduced-calorie diet and regular physical activity, that is part of its licensed use, not optional. If appetite suppression has made it hard to eat enough protein or has led to low energy, a quick conversation with your prescriber can help adjust the plan without changing the dose.
Third, think about the longer picture. If you are wondering how long you should use Wegovy, that is a clinical question your prescriber reviews at each stage, because treatment is not indefinite by default and the goal is sustainable weight management rather than the fastest possible drop. If you are considering starting treatment and want to understand how long you should be on Wegovy for weight loss, checking your eligibility through a free consultation is the clearest first step, as a prescriber can set realistic expectations for your specific situation, including how long you should be on Wegovy given your individual health profile and weight-loss goals.
