Start journey Learn moreThere is no fixed maximum duration set by Mounjaro's UK licence for weight management. How long you stay on it is a clinical decision made with your prescriber, based on how well you're responding and whether the benefits continue to outweigh any risks. Most clinical guidance frames it as a long-term treatment rather than a short course. Mounjaro (tirzepatide) is a prescription-only medicine; a GPhC-registered prescriber assesses suitability at every stage, including whether to continue.

Mounjaro is licensed in the UK for long-term weight management alongside a reduced-calorie diet and increased physical activity. The Summary of Product Characteristics does not set a hard treatment ceiling the way some medicines do. That design reflects the underlying condition: obesity and weight gain are driven by biology, and many people find that their appetite and food-noise return once the medicine is stopped.
Your prescriber will be looking at two things at each review. First, are you still responding? Meaningful weight loss should be visible within the first few months; the clinical threshold used in NICE's appraisal of tirzepatide is less than 5% body-weight reduction after six months at the highest dose you've tolerated, if you're below that, continuing is worth questioning. Second, is your health picture stable? Blood pressure, any underlying conditions, how you're tolerating the medicine, all of it matters, not just the number on the scales.
A question our prescribers hear most weeks is whether staying on Mounjaro long-term is safe. The honest answer is that the trial programme (SURMOUNT-1 ran for 72 weeks and involved thousands of adults) gives a good picture of outcomes over that window, and post-market surveillance continues as part of Mounjaro's Black Triangle status under the MHRA's additional monitoring scheme. No medicine is risk-free, and that's precisely why each repeat order at nume goes back to a prescriber for review, not through an automated renewal.
This is where many people get a surprise. Stopping Mounjaro is not simply a case of finishing a course. Studies of GLP-1 and dual-agonist medicines consistently show that a significant proportion of weight lost during treatment returns within twelve months of stopping, particularly if the lifestyle changes haven't become self-sustaining. The decision about when to stop involves the same prescriber assessment as starting: is your weight stable enough to maintain without pharmacological support, and do you have the dietary and activity patterns to hold it?
This doesn't mean everyone stays on Mounjaro indefinitely. Some people reach a weight they're comfortable with, build habits that genuinely hold, and come off treatment with clinical support. Others find that maintaining even a modest improvement in a weight-related condition (blood pressure, sleep apnoea, joint load) justifies continuing. The length of treatment is as individual as the dose.
What the NHS's NICE appraisal for tirzepatide (TA1026) does state is that if less than 5% weight loss has occurred after six months at the highest tolerated dose, the case for continuing should be actively reviewed. That benchmark is worth knowing, because it frames duration not as a calendar question but as a response question. For more detail on the appraisal criteria, the NICE tirzepatide guidance (TA1026) is publicly available and worth reading alongside any conversation with your prescriber.
A lot of the confusion around duration comes from conflating two different medicines. Wegovy (semaglutide) is recommended by NICE under TA875 for up to two years, and only within a specialist multidisciplinary weight management service. That two-year cap is specific to Wegovy's NHS pathway under TA875, it is not a universal rule for all GLP-1 or GLP-1/GIP medicines.
Mounjaro's NICE appraisal (TA1026, published December 2024, updated September 2025) does not carry that same maximum duration. The NHS medicine information page for tirzepatide confirms it is used as an ongoing treatment, with duration determined by clinical response and tolerability. On a private pathway, your prescriber applies the same clinical judgement without the NHS commissioning constraints, though the same response thresholds are good medicine regardless.
If you'd like to understand how quickly the medicine typically begins working before thinking about duration, the section on how long Mounjaro takes to show results covers the evidence in detail. Costs across different providers and what a private prescription includes are explained on the weight-loss treatments page.
In practical terms, Mounjaro is prescribed month by month on a private pathway. Each repeat requires clinical re-assessment, not a rubber stamp. Evidence of your current dose, weight trajectory and any side effects feeds into whether the prescription is renewed, whether the dose should change, or whether a different approach is more appropriate at this point. That means staying on Mounjaro for two years, three years or longer is clinically possible, but it happens in monthly increments reviewed each time, not as an open-ended standing order.
The broader duration picture for Mounjaro covers the trial timelines and what the data says about weight maintenance over time. And if you're wondering how long reaching a particular target might take, the section on losing 20kg on Mounjaro sets realistic expectations based on the trial evidence.
