Mounjaro storage in practice: what the guidance actually says and why it matters

Step 1, refrigerating your pen from the moment it arrives

Your Mounjaro pen should go into the fridge as soon as it is delivered. The licensed storage condition is 2–8°C, which is ordinary domestic fridge territory. If you have ever wondered whether you actually need to keep Mounjaro in the fridge, the short answer is yes, and the pen stays usable at that temperature right up to the expiry date on the label. A question our prescribers hear regularly is whether the fridge door is acceptable, the door is fine for most pens, but temperature there can fluctuate slightly when the door is opened often, so the main body of the fridge is a more consistent spot if you want to be careful.

The pen must not be frozen. Ice crystals can damage the formulation in ways that are not visible to the naked eye, so a pen that has accidentally been in the freezer compartment should be treated as unusable. Step 2, taking your pen out of the fridge before injection

Many people take their pen out of the fridge 15–30 minutes before injecting. A cold injection is not harmful, but letting the pen reach room temperature can make the experience more comfortable. This is a personal preference, not a clinical requirement.

Once the pen leaves the fridge, the clock starts on your room-temperature window. The Mounjaro SmPC on the Electronic Medicines Compendium (eMC) states the exact number of days a pen may be stored unrefrigerated, read and keep that section of your Patient Information Leaflet, because it is the authoritative figure for your specific pen. If the pen has been outside the fridge and the window has expired, do not inject it. Dispose of it safely in a sharps container and contact your prescriber about a replacement.

Step 3, keeping Mounjaro safe when you are away from home

Travel is one of the more practical storage questions we see. On a short trip, an insulated cool bag with an ice pack wrapped in a cloth (never direct contact with ice) keeps the pen within a safe temperature range. Longer travel (particularly flights) needs a little more planning: airport security accepts medical injectables, and most airline policies allow you to carry your pen in hand luggage with a prescription letter. A travel cool wallet kept in your carry-on, rather than your checked luggage where temperatures can drop sharply, is the sensible approach.

One detail worth knowing: the room-temperature window the leaflet describes begins from the first time the pen is removed from cold storage, not from the date of manufacture. What happens if storage conditions were not right?

A pen that has been left in a hot car, exposed to direct sun through a window, or kept in a warm bathroom cabinet may be outside the conditions the manufacturer tested.
