Does Wegovy work indefinitely, or does it stop working over time?

Semaglutide does not stop working in the way that some people expect. The common misconception is that the body "gets used to" the medicine and weight loss stalls entirely after a year or so. What does NICE actually say about how long you can stay on Wegovy on the NHS?

The NICE technology appraisal TA875 recommends semaglutide (Wegovy) for use within specialist weight management services for a maximum of two years. That cap applies specifically to NHS commissioning: it reflects the conditions under which NICE assessed the cost-effectiveness of the medicine, not a clinical ceiling on how long the drug is safe or licensed to use. If less than 5% weight loss has occurred after six months at maintenance dose, the guidance says treatment should be reconsidered.

Private prescribing sits outside that two-year NHS limit. Here, the licensed indication and the SmPC govern: semaglutide is authorised for weight management in adults with a BMI of 30 or above, or 27 or above with at least one weight-related condition, alongside a reduced-calorie diet and increased activity. There is no fixed stop date written into the licence. Duration becomes a clinical judgement: is the medicine still producing meaningful benefit? Is it well-tolerated? Has the patient's situation changed? Those questions are answered at each clinical review, not decided once at the start.
