The STEP 1 trial, published in the New England Journal of Medicine, documented gastrointestinal side effects as the most common reason participants reduced or paused semaglutide 2.4mg. Crucially, those effects were most frequent early in treatment and during each upward dose step, not as a permanent feature of the medicine. That pattern maps neatly to what semaglutide actually does in the gut: it activates GLP-1 receptors that slow the rate at which the stomach empties its contents into the small intestine. Food that lingers becomes substrate for anaerobic bacterial fermentation, and hydrogen sulphide is one of the gases that process produces. The result is the distinctive rotten-egg burp that many people notice particularly in the morning or after their first meal of the day.
The practical takeaway from the trial data is that sulfur burps are time-limited at each dose level. They are not a signal that something is going wrong; they reflect the body re-calibrating to a new gastric-emptying rate. For most people, the adjustment at any given dose takes one to two weeks. After that window, burping tends to subside noticeably, until the next titration step, when the cycle can repeat at a milder level because the gut is already partially adapted.
Two weeks is a reasonable mid-range estimate, but the actual duration varies. People who eat a diet high in sulphur-containing foods (eggs, red meat, garlic, onions, broccoli, cauliflower) tend to experience more intense and longer-lasting burps because the bacterial load on any retained stomach content is higher. Eating quickly, having large meals, or lying down soon after eating also keeps food in contact with gastric bacteria for longer.
Hydration matters too. Mild dehydration concentrates gastric contents and can make fermentation more noticeable. Some people find that sipping water steadily through the day, rather than drinking large amounts at once, reduces the frequency. The most effective short-term strategy is adjusting what you eat in the 24–48 hours before and after each weekly injection. Reducing eggs, red meat and raw cruciferous vegetables during that window lowers the available sulphur that bacteria can ferment. Peppermint tea has a long informal reputation for easing GI discomfort, though the evidence is stronger for bloating than for sulphide gas specifically.
