Does one Mounjaro pen really cover a full month?

Yes, four doses, four weeks, one pen. Eli Lilly designed the Mounjaro KwikPen so that each pen contains exactly four pre-set doses at the same strength, whether that strength is 2.5 mg, 5 mg, 7.5 mg or any of the higher options in the licensed range. You inject once a week, on the same day each week if possible, and the pen is finished after the fourth injection.

A question our prescribers hear most weeks is whether the pen can stretch further, whether a fifth dose is somehow left in the device. It cannot. The KwikPen's mechanism is set at manufacture; once the fourth dose is delivered, the pen locks. Trying to administer a fifth injection would not produce a dose, and the attempt is wasted effort. Practically, this means your treatment calendar should align neatly with monthly supply cycles. If you order your next pen too late, you risk a gap between doses; your prescriber can advise on the best reorder timing based on your individual schedule.

How long does 1 pen of Mounjaro last once it has been opened?

Once you have taken your first injection from a pen, the clock starts on an in-use storage window. The Mounjaro SmPC specifies the exact number of days you may keep an opened pen, and the Mounjaro SmPC on the eMC is the authoritative place to look that up, we deliberately do not state the figure here because storage guidance can be updated, and reading the leaflet that came with your specific pen is the safest habit.

What is straightforward: the in-use window is measured from your first injection, not from when you bought the pen or removed it from the fridge. If you inject on a Monday, the window starts that Monday. Missing a dose does not pause the clock. This is one reason why keeping your injection day consistent matters: it makes the four-week schedule predictable and means you are less likely to find yourself approaching the pen's in-use limit with doses remaining.

What affects whether the pen stays usable for all four doses?

Storage is the main variable. Before first use, Mounjaro pens must be kept refrigerated at 2–8°C. Temperature excursions (leaving the pen somewhere warm, or accidentally freezing it) can compromise the medicine inside, meaning a pen may technically contain four doses but not four viable doses. Always check the solution looks clear before each injection; if it appears cloudy, discoloured or contains particles, do not use it.

Does the dose strength change how long the pen lasts?

No. Whether your prescription is for the lowest strength or the highest, one pen still provides four weekly doses, the total number of injections does not change across the dose range. What changes is the amount of tirzepatide per dose, not the count.
