Tiredness after starting or increasing Wegovy follows a fairly predictable shape. It tends to arrive one to three days after an injection, coinciding with the period when semaglutide levels in the body are rising most sharply. For many people it feels like a low-energy, heavy-limbed day rather than an incapacitating illness. The NHS medicines page for semaglutide lists fatigue among the common side effects, alongside the more familiar nausea and digestive changes.
The dose step matters a great deal here. The early 0.25 mg and 0.5 mg stages cause less disruption for most people; the bigger jumps (particularly to 1.7 mg and 2.4 mg) are where tiredness becomes more noticeable. Once the body has had four weeks at a given dose, the fatigue associated with that level usually settles. If you want a fuller picture of whether Wegovy causes fatigue and why it happens, the short answer is that it does for many people but it is rarely permanent. So how long semaglutide fatigue lasts is really a question of how many dose increases are still ahead of you, not just where you are today.
Something that catches people out: eating considerably less than usual (which happens naturally on Wegovy as appetite falls) means taking in fewer calories and sometimes less protein. Both can cause genuine tiredness independent of the medicine. Keeping a small, protein-rich snack by whatever is already part of your morning routine can help steady energy levels through the adjustment period. It is a practical detail worth raising with your prescriber.
Fatigue on Wegovy rarely exists in isolation. Nausea and vomiting that prevent proper eating, and diarrhoea that causes fluid loss, can push ordinary tiredness into something more draining. Practical things that tend to help: drinking enough water (dehydration is the commonest amplifier of fatigue on GLP-1 medicines), prioritising sleep, and reducing strenuous exercise in the first few days after each injection rather than pushing through. There is nothing wrong with a gentler week after a dose increase, your body is adapting. Most people find a workable rhythm by their second or third dose at the new level.
What does not help is ignoring tiredness that is worsening rather than improving, or that arrives with symptoms outside the usual GI cluster. The full side-effect picture for Wegovy is broader than most people realise before they start, and understanding it early makes it easier to distinguish normal adjustment from something that needs attention.
Most Wegovy fatigue is self-limiting and benign. Some patterns are not. Seek urgent medical attention if tiredness arrives alongside severe or persistent stomach pain that extends towards the back, this can be a sign of acute pancreatitis, which the MHRA identified as a known but infrequent serious risk in a Drug Safety Update issued in January 2026. Similarly, fatigue paired with a rapid or irregular heartbeat, difficulty breathing, or swelling of the face and throat suggests an allergic reaction and needs emergency care.
Persistent low mood or mood changes that coincide with starting Wegovy are also worth raising with a GP or prescriber promptly. If you are experiencing unexplained or worsening tiredness, it is also worth reading about how semaglutide can affect the thyroid, since thyroid-related changes can contribute to fatigue and are worth ruling out with your prescriber. Fatigue that has not improved at all after four to six weeks at a stable dose (not after a recent increase, but at a dose you have been on for over a month) is another reason to make contact rather than wait. Your body should have found equilibrium by then.
