Step 2: Dose increases and when results become visible (weeks 5–20)

Wegovy is titrated in roughly four-weekly steps: 0.25 mg, then 0.5 mg, then 1.0 mg, then 1.7 mg, then 2.4 mg, the standard maintenance dose. Your prescriber guides this schedule. Each step up can bring a brief return of early side effects before your body resettles.

By weeks four to eight, most people are seeing the scale move with some consistency. Clothes feel different before the numbers feel dramatic. Energy levels can fluctuate, some people feel lighter and more mobile; others notice fatigue, particularly around dose changes. Both are normal.

Once someone reaches 2.4 mg and has been on it for a full titration cycle, weight loss often continues steadily for several more months. The STEP 1 trial reported an average reduction of around 15% of body weight over 68 weeks at 2.4 mg. That average includes people who lost more and people who lost less, individual results differ meaningfully.

The full weight-loss timeline on Wegovy is not linear. There are often periods where the scale barely moves for a week or two, followed by a shift. These plateaus are normal and don't indicate the medicine has stopped working. Consistency with the injection schedule and with eating habits matters throughout.

NICE's recommendation for semaglutide (TA875) includes a checkpoint: if less than 5% weight loss has occurred after six months at the maintenance dose, continuing treatment should be reviewed. A small number of people reach the first dose increase and feel very little, no nausea, no appetite change, nothing obvious. This doesn't necessarily mean the medicine isn't working biologically, but it's worth noting and raising with your prescriber before the next dose step. Occasionally the absence of any GI response at all is a sign worth exploring.

It's also worth being honest about whether lifestyle changes are running alongside treatment. Wegovy works alongside a reduced-calorie diet and increased activity, not instead of them. The weight-loss response is shaped by both.
