Start journey Learn moreMost people taking Wegovy notice some weight loss within the first four to eight weeks, though meaningful, sustained results typically build over four to six months as the dose increases. In the landmark STEP 1 trial, participants lost an average of around 15% of their body weight over 68 weeks — a figure that reflects the full titration journey, not just the early weeks. Wegovy (semaglutide) is a prescription-only medicine for weight management, which means a prescriber assesses whether it is appropriate for you before any treatment begins.

Wegovy begins at 0.25 mg, a dose designed to let your body adjust rather than to produce rapid weight loss. Most people at this stage notice a modest reduction in appetite and feel full a little sooner than usual. The scales may not move much, and that is expected. Nausea, the most commonly reported early side effect, tends to peak in these first weeks and then settle. The NHS semaglutide page lists these early gastrointestinal effects clearly and explains that they are usually temporary.
Think of this phase as groundwork. You are not meant to be losing significant weight yet; the medicine is finding its footing. Many patients keep the pen in the fridge door alongside everyday items as a straightforward reminder that this is a medical routine, not a crash programme.
From week four, the dose steps up roughly every four weeks: 0.5 mg, then 1.0 mg, then 1.7 mg. This gradual increase is where most people begin to see the scales move in a consistent direction. Appetite suppression tends to strengthen with each step, portion sizes often fall naturally, and the uncomfortable early nausea usually fades. For many people, two to four months in is when the change becomes noticeable to others.
How quickly weight comes off varies considerably between individuals. Starting weight, diet, activity levels and how well the medicine is tolerated all play a role. The published STEP 1 trial data in the New England Journal of Medicine showed that participants who completed 68 weeks lost around 15% of their body weight on average, with loss accelerating once the maintenance dose was reached. If you want a broader picture of what weight loss on Wegovy typically looks like, the evidence is encouraging, and honest about the range of outcomes.
Most people reach the 2.4 mg maintenance dose around week 16 to 20. This is the phase where the trial data were gathered and where the clearest sustained results occur. Weight loss at this stage tends to be steadier rather than dramatic: typically one to two pounds a week in people responding well, then gradually slowing as the body settles at a lower weight.
Six months is the first meaningful review point. Under NICE guidance, if less than 5% weight loss has occurred after six months at the maintenance dose, the clinical picture is reassessed. For those responding well, weight loss can continue up to and beyond 12 months. Reaching the maximum weight loss on Wegovy usually means staying on treatment at the maintenance dose, alongside realistic dietary changes, for the better part of a year.
It is also worth knowing that the question of how long Wegovy takes to work has a different answer depending on what you mean by "work", appetite changes come first; the number on the scales follows. Most people find the early appetite shift reassuring even before the weight has moved.
Individual response varies, and it is not a reflection of how well you are following the programme. Biology matters: genetics, metabolic rate, starting BMI, whether other health conditions are present, and how much of the dose your body can tolerate all influence the pace. Lifestyle factors (protein intake, sleep, activity) contribute alongside the medicine, not instead of it.
If progress feels slower than you expected, that is worth raising with your prescriber rather than adjusting anything yourself. Dosing decisions, including whether to stay longer at a lower dose or move to titration, rest with the clinician who reviewed your case. A broader overview of how long weight loss takes on Wegovy covers some of these individual variables in more detail, and if you are curious about how long before you see weight loss on Wegovy, that page explores the early timeline in practical terms.
