The monthly step-up exists for a practical reason: it dramatically reduces the nausea and gastrointestinal discomfort that can otherwise make treatment hard to sustain. But it has a secondary consequence that surprises some patients, you may feel like nothing much is happening in the first eight weeks. That's expected, not a failure.
The dose a prescriber works toward is the highest your body tolerates well, because the clinical evidence for weight loss is built on reaching and staying at the maintenance level.
