Why is the seven-day gap between doses fixed?

Semaglutide has a half-life of approximately one week in the body, which is precisely why Wegovy was designed as a once-weekly injection. Dosing more frequently than every seven days would increase the concentration of the medicine in your bloodstream beyond what the clinical trials tested, raising the likelihood of side effects without meaningful additional benefit. Dosing less frequently would allow levels to drop further than intended, reducing how well it works.

The seven-day interval is not a rough guide, it is the schedule the medicine was authorised on, based on data from the STEP programme trials, including the STEP 1 trial published in the New England Journal of Medicine, which showed around 15% average weight loss over 68 weeks at the 2.4mg maintenance dose. That result reflects participants who followed the weekly schedule as prescribed. Straying from the interval is the kind of thing that sounds minor but affects how consistently the medicine works.

What happens if the gap between doses is shorter or longer than seven days?

A question our prescribers hear most weeks: "I injected on Monday but I'll be away next Monday, can I do it Sunday?" The short answer is yes, occasionally shifting by a day is manageable, but there are rules around how to do it safely.

If you realise you missed a dose and your next scheduled injection is still more than 48 hours away, take the missed dose as soon as you remember and then continue on your original weekly day. If the next scheduled dose is fewer than 48 hours away, skip the missed dose entirely and carry on from there. Never take two doses to make up for a missed one.

If you want to permanently shift your injection day (say, from Monday to Thursday) leave at least five days since your last injection before taking the dose on the new day, then stick to that day going forward. Does the seven-day rule change as the dose increases?

No. The once-weekly interval applies at every stage of the titration ladder: 0.25mg, 0.5mg, 1.0mg, 1.7mg, and 2.4mg all follow exactly the same seven-day schedule. What changes with each dose increase is the amount of semaglutide delivered, not the frequency. Each strength is typically held for at least four weeks before the prescriber considers moving up, allowing your body time to settle before the next step.

With the newer 7.2mg dose (approved by the MHRA in January 2026 and available as a dedicated single-dose pen from April 2026) the weekly interval is identical. A higher dose does not mean more frequent dosing; it means more medicine in the same weekly injection.

Can I manage the weekly schedule around travel, holidays or a busy week?

Yes, with a little planning. Because you have the same seven-day window each week, you have some natural flexibility about the time of day you inject, morning, afternoon or evening all work, as long as seven days separate each dose. If travel is disrupting your usual routine, injecting the evening before departure rather than the morning of arrival is a practical option many people use.

Storage is worth thinking about too. Wegovy pens must be kept refrigerated between 2°C and 8°C.
