How Much Does Mounjaro Cost on the NHS — and What Are Your Options?

Step 1, Understanding what the NHS will and won't cover right now

NICE recommended tirzepatide (Mounjaro) for NHS use in December 2024 under guidance TA1026, but access is being phased in rather than opened to everyone at once. The current cohort (active from June 2025) covers adults with a BMI of 40 or above who also have four or more of five specific conditions: high blood pressure, dyslipidaemia, obstructive sleep apnoea, cardiovascular disease, and type 2 diabetes. A second cohort, for BMI 35–39.9 with four or more of those conditions, began from June 2026. Lower BMI thresholds apply for some ethnic backgrounds under UK guidance.

Even if you meet the criteria on paper, your GP's practice has to be participating in the new contract, and you'll need to take part in wraparound dietary and activity support as part of the programme. Meeting the numbers does not automatically put a pen in your hand. The broader rollout continues in stages through 2027, which is why so many people are looking at private options in the meantime. If the NHS route is open to you, it is worth exploring first, Mounjaro on the NHS is subject to standard prescription charges (currently £9.90 per prescription in England, or free if you hold an exemption certificate). Step 2, How private Mounjaro pricing actually works

Outside the NHS, Mounjaro is available through GPhC-registered online pharmacies and private clinics, always via a prescription following clinical assessment. Eli Lilly revised Mounjaro's UK list price upward from September 2025 (the highest dose pen's list price rose from around £122 to around £330) and typical private prices since then have ranged roughly £149 to £375 per month depending on the dose and provider, as widely reported following Lilly's announcement.

That range matters because Mounjaro is titrated through six dose strengths (2.5mg to 15mg), and you start at the lowest. What you pay at 2.5mg and what you pay at 10mg or 15mg can differ considerably, and understanding how many clicks each pen contains can help you see exactly what you're getting for your money at each dose. Anyone quoting a single low monthly figure without specifying the dose deserves a second look. One thing worth knowing: the consultation and prescription costs are not always included in the headline price. Some providers list the medicine cost and add clinical fees, delivery charges, and aftercare on top. Reading the small print before committing is worth doing.

Step 3, What a legitimate all-in price should cover

A straightforward way to compare providers is to ask what a single quoted monthly price actually includes. At minimum, a credible service should cover clinical assessment, a prescription issued by a registered prescriber, the medicine itself, and delivery. Aftercare (the ability to contact a clinician if something concerns you after starting) should not be an optional extra for a prescription medicine that requires monitoring.

Step 4, Why 'cheapest' is the wrong question for a prescription medicine

The MHRA has warned publicly and repeatedly about counterfeit weight-loss pens sold without proper clinical oversight. In October 2025, enforcement teams raided a site in Northampton producing fake tirzepatide pens, and around 20 million doses of illegally traded weight-loss medicines worth roughly £45 million have been intercepted. Fake pens have been found to contain insulin in some cases. The MHRA's Yellow Card scheme exists partly so that patients can flag suspect sellers.

The right question is not which service charges least; it is which service provides genuine medicine from the licensed UK supply chain, with a real prescriber's assessment, real monitoring, and real support if something goes wrong. For a treatment that works at the level of hormones and appetite regulation, clinical supervision is not a nice-to-have.
