Tirzepatide's licensed UK schedule starts at 2.5mg for four weeks (a tolerability dose, designed to let your system adjust) then steps to 5mg for a further four weeks before the prescriber considers moving higher. So when you're on 5mg, you're in the early part of the treatment arc, not the peak. Weight loss at the 5mg stage is real but typically modest: most people are still adapting, appetite suppression is becoming noticeable, and GI side effects (nausea, indigestion, loose stools) are often at their highest.
The SURMOUNT-1 trial, published in the New England Journal of Medicine, followed 2,539 adults over 72 weeks; average weight reduction at 15mg reached around 20–21%. The 5mg figures from that programme tell a different story, meaningful early progress, but the bulk of weight loss accumulates at higher maintenance doses. Why does this matter for cost? Because some people see 5mg listed as the "cheapest" Mounjaro pen and assume it's a budget route to the same result. It isn't. It's a step in a clinical process. Expecting 15mg outcomes from a 5mg pen is the wrong frame, and a prescriber who explains that upfront (rather than just processing your order) is worth more than a lower price tag.
Private Mounjaro is priced per pen (each pen contains four weekly doses). In September 2025 Eli Lilly significantly raised UK list prices, at the higher doses, the list price rose from roughly £122 to roughly £330 per four-week pen, as widely reported at the time. The 5mg pen sits lower on the published price ladder, but private providers set their own margins, so the price you see quoted will vary between clinics and pharmacies.
What varies more is what that price includes. Some services list a bare-minimum number because consultation, prescription, delivery and aftercare support are charged separately. Suspect sellers are a pattern worth knowing: no clinical assessment, prices well below market, no verifiable registration. If something is being sold as Mounjaro without a prescription and a qualified review, it is not a bargain, it's a risk.
